Wife of Emperor Monmu. Daughter of Fuhito. In 697 (1st year of Emperor Monmu), she became the wife of Emperor Monmu, surpassing the daughters of the Ishikawa clan. In 701 (1st year of the Taiho era), she gave birth to Emperor Shomu and played an important role in the imperial harem. She laid the foundation for the Fujiwara clan to expand its influence as maternal relatives, and later became empress dowager and grand empress dowager. In 737 (9th year of the Tenpyo era), the monk Genbo, who cured Miyako's illness, gained his favor and wielded power for a time after the four Fujiwara sons (Muchimaro, Fusasaki, Umakai, and Maro) died one after another.
