Hilar lymph node tuberculosis

This is the most common type of early tuberculosis. When tuberculosis bacteria reach the alveoli of an uninfected individual and create a primary infection focus, the bacteria flow along the lymphatic system to the associated lymph nodes before the individual has acquired hypersensitivity or immunity, forming a primary infection lymph node focus. When the disease is active, the mediastinal lymph nodes are also affected, and ultimately the bacteria are more likely to flow into the bloodstream through the right venous angle lymph nodes. If the hilar lymph node enlarges significantly, it may sometimes compress the bronchi in children, causing atelectasis. Caseous hilar lymph node focus may erode the adhered bronchial wall and perforate into the cavity, causing tuberculous pneumonia due to inhalation of caseous material. Hilar lymph node tuberculosis, which was once considered a childhood or adolescent disease, is now seen in adults as the age of primary infection increases, so caution is required. Treatment is similar to that for pulmonary tuberculosis.

もっとも多い初期結核症で、結核菌が未感染の個体の肺胞まで到着して初感染原発巣をつくると、菌は過敏性も免疫もまだ獲得しないうちにリンパの流れにのって所属リンパ節に流入し、初感染リンパ節巣を形成する。病勢の盛んなときは縦隔リンパ節にも連続的に罹患(りかん)し、最終的には右静脈角リンパ節より菌が血流に流入しやすい。肺門リンパ節腫大(しゅだい)が著しいと、小児ではときに気管支を圧迫して無気肺をおこすこともある。肺門リンパ節乾酪化巣が癒着した気管支壁を侵食して腔(くう)内に穿孔(せんこう)し、乾酪物質の吸引による結核性肺炎がおこることもある。かつては小児期や思春期の疾患とされていた肺門リンパ節結核が、初感染年齢の高齢化とともに成人にもみられるようになったので、注意を要する。治療は肺結核に準ずる。
