What is the disease? The meibomian glands (sebaceous glands deep inside the eyelids) become clogged. What is the cause?The meibomian glands have openings near the base of the eyelashes, and this condition occurs when these openings become clogged due to inflammation or other reasons. How symptoms manifestA dome-shaped lump appears on the eyelid. It is usually not painful. If left untreated, it will gradually grow larger. When touched from the outside, it feels like a hard lump. If left untreated, the contents may break and leak out from the skin or inside of the eyelid. Testing and diagnosisDiagnosis is relatively easy based on symptoms and the presence of a painless lump. However, if the lesion recurs repeatedly in the same area in elderly patients, it may be cancer, so the tissue removed by surgery must be examined under a microscope. Treatment methodsLocal anesthesia is administered, and the tumor is removed through an incision. In the case of children, general anesthesia is required. In principle, surgery is required, but if surgery is to be avoided, steroid drugs can be injected into the tumor. If this does not cure the tumor, surgery is still required. What to do if you notice an illnessEven if you notice a lump on your eyelid, it usually doesn't hurt, so people tend to delay seeing an ophthalmologist. It tends to get bigger gradually, so it's best to see an ophthalmologist as soon as possible. どんな病気か マイボーム腺（まぶたの深部にある脂腺）が詰まって、なかに 原因は何かマイボーム腺はまつ毛の生え際近くに開口部がありますが、その開口部が炎症などで詰まることによって起こります。 症状の現れ方まぶたにドーム状のしこりができます。普通、痛みはありません。放置すると次第に大きくなっていきます。外から触れるとコリコリしたしこりができています。 放置すると、まぶたの皮膚側や内側に破れて内容物が出ることがあります。しかし内容物が出ても、 検査と診断症状と無痛性のしこりの存在から、診断は比較的容易です。ただ、高齢者で同一部に再発を繰り返す場合、がんの可能性もあるので、手術で摘出した組織を顕微鏡で調べる必要があります。 治療の方法局所麻酔をし、切開して摘出します。小児の場合は全身麻酔が必要になります。原則は手術ですが、手術を避けたい場合は、ステロイド薬を腫瘤に注射します。これで治らない場合は、やはり手術が必要です。 病気に気づいたらどうするまぶたにできたしこりに気づいても、通常は痛くないので、眼科受診が遅れがちです。傾向としては徐々に大きくなっていくので、早めに眼科を受診しましょう。 森 秀夫 出典 法研「六訂版 家庭医学大全科」六訂版 家庭医学大全科について 情報 |
