[What kind of disease is it?]
It is a sexually transmitted disease caused by the bacterium Haemophilus ducreyi (chancroid).
Two to three days after the sexual intercourse that led to the infection, or in slow cases around a week, several ulcers the size of a soybean appear near the coronary sulcus of the penis (glans sulcus) or inside the foreskin in men, and on the labia or vaginal vestibule in women.
Ulcers are round or oval in shape, have a clearly defined border, contain dirty pus on the surface, are soft to the touch, and are extremely painful.
There are many Haemophilus ducreyi inside the ulcer, and they attach to the surrounding area together with the secretions, causing new ulcers to spread.
One to two weeks after the ulcers form, nearly half of the patients develop swelling and pain in the inguinal lymph nodes, and if the ulcers suppurate, they develop a fever. Chancroid and syphilis can occur together, in which case the ulcers gradually harden and turn into hard chancroid (the first type of syphilis).
This is called mixed chancroid, but if you treat chancroid, the symptoms of mixed chancroid do not appear and it takes the form of latent syphilis, so the concomitant syphilis may go unnoticed. For this reason, a syphilis serology test (column "Syphilis serology") must be performed 6 weeks after the diagnosis of chancroid.
[Treatment]
Usually, a sulfa drug is taken orally for 1 to 2 weeks. Streptomycin, tetracycline, or chloramphenicol may also be taken orally or by injection.
For ulcers, an ointment containing a sulfa drug or tetracycline is spread onto gauze and applied.

[どんな病気か]
　性感染症の1つで、ヘモフィリス・デュクレイ（軟性下疳菌）という細菌の感染でおこります。
　感染機会となった性行為から2～3日、おそい場合は1週間前後で、男性はペニスの冠状溝（かんじょうこう）（亀頭溝（きとうこう））付近や包皮（ほうひ）の内側、女性は陰唇（いんしん）や腟前庭（ちつぜんてい）に、大豆（だいず）くらいの大きさの潰瘍（かいよう）が数個発生します。
　潰瘍は、円形か楕円形（だえんけい）で、周囲との境界がはっきりしていて、表面に汚い膿（うみ）をもち、さわるとやわらかく、激しく痛みます。
　潰瘍の中には、たくさんのヘモフィリス・デュクレイがいて、分泌物（ぶんぴつぶつ）といっしょに周囲にくっつき、新しい潰瘍が広がっていきます。
　潰瘍ができて1～2週間後に、半数近くの人は、鼠径（そけい）リンパ節（せつ）の腫（は）れと痛みがおこり、化膿（かのう）すると熱が出ます。軟性下疳と梅毒（ばいどく）がいっしょにおこることがあって、この場合は、潰瘍がしだいにかたくなり、硬性下疳（こうせいげかん）（「梅毒」の第1のようになります。
　これを混合下疳といいますが、軟性下疳の治療を行なうと混合下疳の症状が現われず、潜伏梅毒のかたちをとるので、梅毒の併発に気づかないことがあります。このため、軟性下疳の診断から6週間後に梅毒血清反応（ばいどくけっせいはんのう）検査（コラム「梅毒血清反応」）を必ず行ないます。
[治療]
　ふつう、サルファ剤を1～2週間、内服します。ストレプトマイシン、テトラサイクリン、クロラムフェニコールなどを内服や注射で使用することもあります。
　潰瘍には、サルファ剤やテトラサイクリン含有の軟膏（なんこう）をガーゼにのばして貼（は）ります。
