Bile duct dilation - cholangiectasis

The bile duct is a bile excretory pathway that connects the liver with the gallbladder and duodenum, and is made up of the intrahepatic bile duct, common bile duct, and gallbladder duct. In normal adults, the diameter of the bile duct is 8 mm or less. However, when bile secretion is impeded due to complete or incomplete obstruction of the biliary system or dysfunction of the gallbladder or papilla of Vater, the internal pressure rises and the bile duct expands, and in extreme cases, the diameter can reach 20 mm or more. This condition is called bile duct dilation. Therefore, when dilation is found by cholangiography, it is an important finding in diagnosing diseases of the biliary tract and pancreas, and diseases such as bile duct stones, bile duct cancer, pancreatic head cancer, chronic pancreatitis, Vater papillitis/cancer, and gallbladder dysfunction are suspected.

胆管は，肝臓と胆囊，十二指腸を結ぶ胆汁排出路であり，肝内胆管，総胆管，胆囊管などからなる。その太さは，正常成人では径8mm以下である。しかし，胆道系の完全または不完全な閉塞や，胆囊，ファーター乳頭の機能障害により，胆汁分泌が障害されると，内圧が上昇し，胆管が拡張し，極端な場合には，径20mm以上に及ぶ。この状態を胆管拡張という。したがって，胆道造影によりその拡張をみつけた場合は，胆道，膵臓の病気を診断する重要な所見となり，胆管結石，胆管癌，膵頭部癌，慢性膵炎，ファーター乳頭炎･癌，胆囊機能不全などの病気が疑われる。
