An abscess formed when pus accumulates in the Douglas pouch. The Douglas pouch is the bottom part of the abdominal cavity, between the rectum and bladder in men and between the rectum and uterus in women. It is named after the Scottish obstetrician J. Douglas (1765-1742). Because the Douglas pouch is the bottom part of the abdominal cavity, it is easy for pus to accumulate in the abdominal cavity. The main causes are appendicitis and inflammation of the female genitals. Symptoms include lower abdominal discomfort, lower abdominal pain during defecation, diarrhea with mucus, tenesmus, frequent urination, painful urination, vaginal discharge, and genital bleeding. The pus may be absorbed naturally or may be discharged to the rectum, but it can leave aftereffects such as adhesions of the intestines and fallopian tubes and abnormal positioning of the uterus. In general, chemotherapy is not very effective, and it is necessary to drain the pus by incising the anterior wall of the rectum or the posterior wall of the vagina. ダグラス窩に膿がたまって膿瘍を形成したもの。ダグラス窩とは腹腔の最底位の部分で,男性では直腸と膀胱の間,女性では直腸と子宮の間をいう。スコットランドの産科医 J.ダグラス (1765~1742) にちなむ名称。ダグラス窩は腹腔の最底部なので,腹腔内の膿がたまりやすい。虫垂炎と女性性器の炎症がおもな原因になる。症状は下腹部の不快感,排便時の下腹部痛,粘液の混った下痢便,しぶり腹,頻尿,排尿痛,帯下,性器出血など。自然に膿が吸収されたり,直腸などに出てなおることもあるが,腸管や卵管の癒着,子宮の位置異常などの後遺症を残す。一般に化学療法の効果が少く,直腸の前壁や腟の後壁を切開して排膿する必要がある。
