Ox's eye

This refers to glaucoma in infants. Until the age of three, the eyeball wall is soft, and if glaucoma develops at this age, which causes an increase in intraocular pressure, the eyeball grows larger and the cornea also grows, giving it a cow's eye appearance. It is caused by congenital abnormalities in the formation of the angle and inflammation during fetal development that impairs the outflow of aqueous humor. Previously, it was collectively called congenital glaucoma, but since 2003 it has been called developmental glaucoma (Japan Glaucoma Society). It is often noticed when the cornea turns white like ground glass or when the patient has strabismus. Microscopic surgery, which involves incising the angle wall, is effective immediately, and the earlier the better the results. After surgery, treatment for amblyopia is required. Some patients have a large cornea but no increase in intraocular pressure, and there are no other problems, so caution is required.

乳幼児の緑内障をいう。3歳までは眼球壁が軟らかく、このころに眼圧が上昇する緑内障にかかると、眼球が膨らむように大きく発育し、角膜も大きくなって牛の目のような外観になる。先天性の隅角形成異常や胎児期の炎症による房水流出障害などが原因となる。従来、先天緑内障と総称されていたが、2003年（平成15）より発達緑内障とよぶことになった（日本緑内障学会）。角膜が磨(すり)ガラス状に白くなったり、斜視などから気づかれることが多い。隅角壁の切開という顕微鏡手術が即効的であり、早いほど結果がよい。術後は弱視治療が必要となる。なかには角膜が大きいだけで眼圧上昇もなく、なんら障害のない巨大角膜があるので注意を要する。
