What is the disease?This is a disease that develops by eating fish (such as mackerel, Alaska pollock, horse mackerel, and Japanese flying squid) that are infested with the larvae of Anisakis species (such as Anisakis and Pseudoterranova). These parasites live in marine mammals (such as dolphins). When infected Anisakis larvae penetrate the stomach mucosa, the disease is called gastric anisakiasis. How symptoms manifestWithin a few hours after ingestion, symptoms such as sudden upper abdominal pain and vomiting appear. The abdominal pain is thought to be due to allergic symptoms rather than mechanical irritation caused by the larvae penetrating the gastric mucosa. In rare cases, the patient may fall into anaphylactic shock (a life-threatening condition that affects the whole body, such as sudden difficulty in breathing and a drop in blood pressure). Testing and diagnosisIt can be easily diagnosed by checking whether fresh fish was eaten within a few days of the onset of symptoms, and by specifically checking for the parasite by gastrointestinal endoscopy. In many cases, a swelling where the parasite has penetrated the mucous membrane can be confirmed. Treatment methodsIf the parasite is confirmed by gastroscopy, it will be removed on the spot. Even if it is not confirmed, if the patient has eaten fresh squid and this disease is strongly suspected, symptoms will improve with the administration of anticholinergic drugs, antihistamines, and steroid drugs, and the prognosis will be good. What to do if you notice an illnessIf you suddenly experience abdominal pain or vomiting after eating fresh fish, do not panic and visit a medical facility that can perform an endoscopic examination. If you cannot visit a medical facility immediately, such as at night, you may visit the next day, depending on the severity of your symptoms. Humans are not suitable hosts for Anisakis. Therefore, they do not develop into adults in the human digestive tract, but anisakiasis is an important parasitic disease because of the various symptoms it causes. It is a disease closely related to the Japanese food culture, which favors fresh seafood, and the most common sources of infection are marinated mackerel and squid. どんな病気かアニサキス類(アニサキス、シュードテラノーバなど)の幼虫が寄生した魚類(マサバ、スケソウダラ、マアジ、スルメイカなど)を食べることによって発症する病気です。 これら寄生虫は、海の哺乳動物(イルカなどの 感染したアニサキス類の幼虫が、胃の粘膜へ入り込んで発症する場合を胃アニサキス症といいます。 症状の現れ方食後数時間以内に、急激な上腹部痛や嘔吐などの症状で発症します。腹痛は、幼虫の胃粘膜への刺入による機械的刺激よりも、むしろアレルギー症状によると考えられています。まれに、アナフィラキシーショック症状(急激な呼吸困難や血圧低下など全身的な生死に関わる症状)に陥ることもあります。 検査と診断症状の起こる数日以内に新鮮な魚類を食べたかどうか、具体的には胃内視鏡検査で虫体を確認することで容易に診断できます。多くの場合、虫体が粘膜に刺入した盛り上がりが確認できます。 治療の方法胃内視鏡検査で虫体が確認されれば、その場で摘出します。確認できなくても、新鮮なイカなどを食べていて、この病気が強く疑われる場合は、抗コリン薬、抗ヒスタミン薬、ステロイド薬の投与によって症状は軽快し、予後は良好です。 病気に気づいたらどうする新鮮な魚類を食べたあとに、突然の腹痛や嘔吐が起きたら、あわてずに内視鏡検査の可能な医療施設を受診してください。夜間など、すぐに受診できない場合は、症状の程度にもよりますが、翌日に受診してもよいでしょう。 ヒトは、アニサキス類の好適宿主ではありません。そのため、ヒトの消化管では成虫に発育することはありませんが、いろいろな症状を示すことより、アニサキス症は重要な寄生虫病になっています。生鮮魚介類を好む日本人の食文化と密接な関係がある病気で、最も多い感染源はシメサバやイカです。 桑山 肇 出典 法研「六訂版 家庭医学大全科」六訂版 家庭医学大全科について 情報 |
