Bartholin's gland

A cherry-sized secretory gland located on either side of the vaginal entrance in the vaginal vestibule. Its duct is about 2 cm long and opens inside the labia minora, just before the hymen. During sexual arousal, the bulbocavernosus muscle surrounding the gland contracts to secrete a colorless, transparent mucus, which acts as a lubricant during sexual intercourse. Bartholin's gland inflammation is an inflammation caused by bacterial infection of the duct. As the condition progresses, the duct becomes blocked, pus accumulates inside the gland cavity, and a Bartholin's gland abscess forms. A spherical swelling is palpable on either the left or right side of the vaginal entrance, and the area is tender. Severe pain is felt when walking, sitting, urinating, or defecating. If the condition does not improve with antibiotics, the entire mass must be removed by incision and drainage or surgery.

腟前庭(ちつぜんてい)で腟入口の左右にあるサクランボ大の分泌腺。その導管は2センチメートルくらいの長さで、小陰唇の内側で処女膜の直前に開口している。性的興奮時に腺を囲む球海綿体筋の収縮によって無色透明の粘液を分泌し、これが性交の潤滑液の役目をする。バルトリン腺炎はこの導管部から細菌が感染しておこる炎症で、進行するとこの導管が閉塞(へいそく)して腺腔(せんくう)内部に膿(のう)が貯留し、バルトリン腺膿瘍(のうよう)が形成される。腟入口の左右のいずれか一側に球状の腫脹(しゅちょう)を触れ、その部に圧痛がある。歩行、起坐(きざ)、排尿、排便時に激しい痛みを感ずるようになる。抗生物質の服用によって軽快しない場合には、切開排膿あるいは手術によって腫瘤(しゅりゅう)全体を摘出する必要がある。
