A type of tuberculous rash (→ cutaneous tuberculosis). It mainly occurs in women on the lower legs, and sometimes on the forearms, as multiple dark red subcutaneous nodular lesions up to the size of a pigeon's egg. Usually, no systemic symptoms are present, but if the disease develops after primary tuberculosis infection, it may be accompanied by fever and joint pain. The nodular lesions later soften and form fistulas through which the contents may be discharged to the skin surface. Histologically, it is accompanied by necrosis and degeneration of the deep dermis and subcutaneous tissue, and is a granuloma in which epithelioid cells and Langhans giant cells are proliferated, and sometimes there is vascular lesion (arteriovenitis). The relationship between this disease and tuberculosis is not necessarily clear, and circulatory disorders are said to be involved. In the United States, it is often called nodular vasculitis. A. 結核疹 (→皮膚結核 ) の一つ。おもに女性の下腿,ときに前腕に暗赤色の鳩卵大ぐらいまでの皮下結節性病変が多発する。通常,全身症状を伴わないが,結核初感染後に発病した場合は,発熱,関節痛などを伴うことがある。結節性病変はのちに軟化し,瘻孔を形成して,内容物が皮表に排出されることがある。組織学的には真皮深層,皮下組織の壊死,変性を伴い,類上皮細胞や,ラングハンス巨細胞の増殖を認める肉芽腫で,ときに血管病変 (動静脈炎) がある。本症の発病には結核との関係は必ずしも明確ではなく,循環障害が関与するといわれる。アメリカでは結節性血管炎と呼ぶことが多い。 A.バザンはフランスの皮膚科医 (1807~1878) 。
