Intermediate facility - Chukan Shisetsu

Japanese: 中間施設 - ちゅうかんしせつ

A facility that is intermediate between a medical facility and a welfare facility for the elderly, or between the home and these. In the field of geriatric medical care, it refers to a facility for those who need care and cannot be accommodated by day care services, short-term admission facilities such as short stays, or home care services. It began as a proposal by the Social Security System Council in 1985. At that time, many elderly people who did not need hospitalization were hospitalized for long periods under the pretext of social hospitalization, which led to huge medical expenses for the elderly, and there were concerns about a future crisis in medical finances. The purpose of this was to establish a facility that could act as a bridge to help such elderly people smoothly return and adapt to life at home and in the local community, thereby improving the foundation of social security and the financial situation (→ Geriatric Health Care Facilities). In the field of psychiatric care, psychiatric intermediate facilities established in the late 1950s are an example of this.

医療施設と老人福祉施設，あるいはこれらと家庭との中間的な位置づけの施設。老人医療分野において，デイケアサービス，ショートステイなどの通所・短期入所施設や在宅ケアサービスでは対応できない要介護者のための入所施設をいう。1985年の社会保障制度審議会における提言に始まる。当時は入院治療を必要としない多くの高齢者が社会的入院の名目で長期間入院していたために老人医療費が膨大となり，将来の医療財政の破綻危機が懸念されていた。そうした高齢者が，在宅生活や地域社会にスムーズに復帰・適応できるよう橋渡しをする施設を設けることで，社会保障の基盤整備や財政状況の改善につなげることを目的とした（→老人保健施設）。また精神医療分野では，1950年代後半から設置された精神科中間施設がこれにあたる。
