Chapped skin

A colloquial name for fissured eczema. This is a skin disease that is an aggravation of cracks (scaling eczema). Like cracks, it is prevalent in children and housewives, with the hands, feet, and face seen in children and the hands of housewives. The main causes of cracks in children are cold weather and cold winds, and housewives doing housework, especially using detergents and soaps. First, the skin becomes dry and the outermost layer of the stratum corneum peels off and becomes flaky. At this stage, when the condition is called cracks, if the condition progresses further, the skin becomes thick and hard, linear skin grooves become noticeable, and eventually the elasticity of the skin decreases and cracks appear, resulting in a condition called chapped skin. Usually, patients complain of a slight itchiness, but if the cracks are deep, pain and bleeding may occur. Chapped skin caused by using soap and detergents in housewives is also known as housewives' eczema. Treatment involves applying petrolatum, urea ointment, ointment containing heparin-like substances, steroid ointment, or ointment containing 5-10% salicylic acid. For housewives, it is advisable to refrain from doing wet chores for a while before treatment, but if this is not possible, wear rubber or vinyl gloves over cotton gloves to prevent soap or detergent from coming into direct contact with the skin.

亀裂(きれつ)性湿疹(しっしん)の俗称。ひび(落屑(らくせつ)性湿疹)の悪化した皮膚病で、ひびと同様に幼小児と主婦に好発し、小児では手足と顔面、主婦では手にみられる。また、小児では寒冷や寒風、主婦では水仕事、とくに洗剤やせっけんの使用がそれぞれおもな原因となる。まず皮膚が乾燥して、表皮の角質層の最表面がはがれてかさかさしてくる。この病状がひびとよばれる段階で、さらに病状が進むと、皮膚が厚く硬くなり、線状の皮溝が目だち、やがて皮膚の弾力性が低下して亀裂を生じ、あかぎれとよばれる症状となる。普通、軽いかゆみを訴えるが、亀裂が深いと痛みがおこり、出血することもある。せっけんや洗剤の使用による主婦のあかぎれは、主婦湿疹としても知られる。治療にはワセリン、尿素軟膏(なんこう)、ヘパリン様物質含有軟膏、ステロイド軟膏、あるいは5~10%サリチル酸含有軟膏を塗布する。主婦の場合、水仕事をしばらくしないで治療するのが望ましいが、それができない場合は、木綿の手袋をはめた上にゴムまたはビニール手袋を用い、せっけんや洗剤が直接皮膚に触れないようにする。
