This is a document that a doctor or dentist must issue to a patient or the person currently caring for the patient when they recognize the need to dispense and administer a drug to the patient for medical purposes. The obligation to issue a prescription is stipulated in the Medical Practitioners Act and the Dentist Act, and a prescription must state the patient's name and age, the name of the drug, amount, usage, dosage, date of issue, period of use, the name and location of the hospital or clinic or the address of the doctor, and must be signed and stamped by the prescribing doctor. According to the Pharmacists Act, pharmacists are to dispense medicines according to prescriptions from doctors, dentists, or veterinarians, but if there are any doubts about the contents of the prescription, they are required to inquire about the matter with the doctor who issued the prescription. In addition, pharmacy operators must keep dispensed prescriptions for three years from the date of dispensing. A prescription is also required for medical equipment such as glasses. There is no standard method for writing the name of the drug, the amount, the usage, and the dosage on a prescription, and it varies from facility to facility or from doctor to doctor, and this has been pointed out as one of the causes of medical accidents. In response to this, the Ministry of Health, Labor and Welfare ("Study Group on the Method of Writing Oral Drug Prescriptions") is working to standardize the method of writing prescriptions.
