Construction began on the Mie and Wakayama sides, with the section between Aikaguchi (now Taki) and Tochihara opening as the Kisei East Line in 1923, and the section between Wakayama and Minoshima opening as the Kisei West Line in 1924. It was subsequently extended and opened in stages, and completed in 1959 with the opening of the Miki-sato-Shika section. The entire line was renamed the Kisei Main Line when the Sangu Line section between Kameyama and Aikaguchi was added.
