Aldosteronism

This disease is caused by the excessive secretion of aldosterone, a type of adrenal cortex hormone. It is also called Conn syndrome, named after the discoverer who first described the disease in 1955. Aldosterone acts on the kidneys to promote sodium reabsorption and potassium excretion, so when it is secreted in large quantities, sodium and water are retained in the body, causing high blood pressure and a decrease in serum potassium. In addition, the pH of the blood (acid-base balance) becomes alkaline, resulting in metabolic alkalosis. Therefore, in addition to high blood pressure, symptoms include excessive drinking and urination due to hypokalemia, periodic paralysis in which the arms and legs become immobile for several hours, and tetany attacks in which the fingers become bent and immobile due to metabolic alkalosis. If left untreated for a long period of time, it can cause heart failure. The most common cause is a tumor in the adrenal gland, which is usually a benign adrenal adenoma, and rarely due to adrenal cancer. It is more common in women, twice as common as in men. In terms of age, it is most common in people in their 30s, followed by those in their 40s, with both accounting for 80% of cases. Another cause is hyperplasia (an increase in tissue components, resulting in the enlargement of tissue and organs) in both adrenal glands. Diagnosis requires various stress tests, including elevated blood aldosterone levels, decreased plasma renin activity, hypokalemia, and metabolic alkalosis, to prove that blood renin activity is not elevated. In addition, to determine the location of the tumor, abdominal CT (computed tomography), adrenal scintigraphy using isotopes, or venous sampling by inserting a catheter into the adrenal vein are performed. The best treatment is surgical removal of the adenoma (laparoscopic surgery is now possible). Hyperplasia is prone to recurrence.

副腎(ふくじん)皮質ホルモンの一種であるアルドステロンが大量に分泌されるためにおこる疾患。1955年に初めてこの病気を記載した発見者の名をとってコーンConn症候群ともよばれる。アルドステロンは腎臓に作用してナトリウムの再吸収およびカリウムの排泄(はいせつ)を促進させるので、大量に分泌されるとナトリウムや水が体内に貯留して高血圧となり、血清カリウムが低下する。また、血液のpH（酸塩基平衡）がアルカリ側に偏り代謝性アルカローシスとなる。したがって症状としては、高血圧のほか、低カリウム血症による多飲や多尿、発作的に数時間手足が動かなくなる周期性四肢麻痺(まひ)をおこしたり、代謝性アルカローシスによって手指が曲がったまま動かなくなるテタニー発作をおこすことがある。長期間放置すると、心不全を引き起こす。原因は副腎にできる腫瘍(しゅよう)による場合がもっとも多く、普通は良性の副腎腺腫(せんしゅ)で、副腎癌(がん)によるものはまれである。女性に多く、男性の2倍である。年齢的には30代にもっとも多く、ついで40代、両者で80％を占める。ほかに両側の副腎にみられる過形成（組織を構成する成分が増加し、その組織や臓器が大きくなること）によるものがある。診断にはアルドステロンの血中濃度上昇、血漿(けっしょう)レニン活性の低下、低カリウム血症、代謝性アルカローシスをはじめ、諸種の負荷テストを行い、血中レニン活性が上昇しない点を証明することが必要である。また、腫瘍の部位を決めるためには、腹部CT（コンピュータ断層撮影）、アイソトープを使った副腎シンチグラフィあるいは、副腎静脈にカテーテルを入れて静脈サンプリングを行う。治療は手術（腹腔(ふくくう)鏡手術が可能となっている）によって腺腫を摘出するのがもっともよい。過形成によるものは再発しやすい。
