What is the disease?

It is a soft wart that is commonly seen in childhood. It can also appear on the genitals of adults as a sexually transmitted disease.

What is the cause?

It is caused by infection with the molluscum contagiosum virus. It is transmitted by contact and is more likely to spread when the skin barrier function is impaired due to atopic dermatitis or other factors.

How symptoms manifest

Single or multiple lesions measuring 1 to 5 mm in diameter Papule ( Emergency ) , Nodules ( Presentation ) They appear on the trunk and limbs. They are covered with normal or light pink skin and are hemispherical in shape. Umbilical ( Last update ) The area may become sunken (like a belly button). There are usually no symptoms, but inflammation may cause itching.

Testing and diagnosis

The diagnosis is easy if the patient has small dome-shaped papules with a moist appearance. Porridge-like ( College ) The contents are sampled and examined, or characteristic inclusion bodies formed within the epidermis are observed histopathologically.

Treatment methods

At present, there are no safe and effective antiviral drugs for this disease. Statistically, it has been reported that the viruses will naturally fall off after about six months on average if left untreated, but they may persist for years and increase in number.

To eliminate discomfort and to prevent the spread of the disease to others, forceps ( guard ) Removal surgery is often performed to pinch out the contents. In most cases, the decision to remove the tumor is made with the understanding that it will naturally disappear.

In the case of this sexually transmitted disease, the first choice is removal using forceps.

What kind of infection is it?

It is caused by infection with the molluscum contagiosum virus, which belongs to the genus Morsipoxvirus of the Poxvirus family. It is also known as "water warts." It generally occurs in children, especially those with atopic dermatitis, and is often contracted in swimming pools.

It can also occur frequently on the genitals of adults as a sexually transmitted infection (STI).

How symptoms manifest

Treatment methods

どんな病気か

　小児期によくみられる皮膚の軟らかいいぼです。性行為感染症として、成人の陰部にみられることもあります。

原因は何か

　伝染性軟属腫ウイルスの感染によって発症します。感染様式は接触感染で、アトピー性皮膚炎やそのほかの要因により皮膚のバリア機能に障害がある時に感染しやすいといえます。

症状の現れ方

　単発あるいは多発性に認められる径1～5㎜ほどの丘疹(きゅうしん)、小結節(しょうけっせつ)が体幹、四肢に現れます。通常の色あるいは淡紅色の皮膚でおおわれ、半球状に隆起しています。発達したものでは中心が臍窩状(さいかじょう)に（へそのように）陥没することがあります。自覚症状はないことが多いのですが、炎症などによりかゆみを伴うこともあります。

検査と診断

　臨床的にみずみずしい外観のドーム状小丘疹があれば、診断は容易です。ほかの病気との区別が必要な場合は、粥状(じゅくじょう)の内容物をつまみ出して確認するか、病理組織学的に表皮内に形成される特徴的な封入体を観察します。

治療の方法

　現在のところ、本症に対する安全かつ有効な抗ウイルス薬は存在しません。統計的には、放置しても平均して約半年の経過で自然に脱落すると報告されていますが、年余にわたって存在し、数が増えることもあります。

　不快感を取り除くため、また、ほかへの伝搬を予防する意味でも、鉗子(かんし)などで内容物をつまみ出す摘除術もよく行われます。多くは自然消退することを念頭において、摘除するかどうか決定します。

　性行為感染症としての本症では、鉗子などによる除去を第一選択として行います。

どんな感染症か

　ポックスウイルス科モルシポックスウイルス属の、伝染性軟属腫ウイルスの感染によります。いわゆる「みずいぼ」のことです。一般に小児、とくにアトピー性皮膚炎の小児によく起こり、プールで感染することが多くみられます。

　また、性行為感染症（STI）として、成人の性器に多発することがあります。

症状の現れ方

　主に、毛包(もうほう)から感染します。2～7週の潜伏期ののちに、粟粒(ぞくりゅう)大～大豆大（1～数㎜大）までの、中央に凹みのあるドーム状の腫瘍が、体幹部や四肢に多発します。

　表面は平滑で、白色～皮膚色で蠟(ろう)のような光沢があり、数個あるいは無数に散在したり集まったりしています。周囲が湿疹化し、かゆみを伴うことがあります。

検査と診断

　ピンセットでつまむと、乳白色の粥状(じゅくじょう)物質が押し出されるのが特徴です。ひとつだけの場合、いぼや稗粒腫(はいりゅうしゅ)（眼のまわりの小さな白っぽいできもの）との区別が必要で、切除して組織像で診断します。

治療の方法

　ピンセットで一つ一つ摘み取るのが最も確実な方法です。液体窒素(ちっそ)による凍結療法や、硝酸銀(しょうさんぎん)などの腐食剤などで取る方法もあります。みずいぼは数カ月から数年、持続しますが、自然に、または外傷や細菌感染をきっかけに治ります。再感染もしばしば起こります。

病気に気づいたらどうする

　みずいぼが増えない前に、できるだけ早く皮膚科を受診しましょう。
