Painkillers. Commonly known as painkillers, these are drugs that act on the central nervous system to relieve or reduce pain without causing loss of consciousness. There are narcotic and non-narcotic analgesics. Narcotic analgesics include natural narcotics made from opium and synthetic narcotics made purely chemically. Non-narcotic analgesics include pentazocine and buprenorphine hydrochloride, as well as antipyretics and anti-inflammatory analgesics such as acetaminophen.

Narcotic analgesics

(1) Natural narcotics The most effective analgesic is said to be morphine hydrochloride, an opium alkaloid preparation. It is addictive, habit-forming, and causes withdrawal symptoms, so it is regulated by the Narcotics and Psychotropic Substances Control Law. Another drawback is that it can cause respiratory paralysis. Other opium alkaloid preparations include opium powder, opium powder, opium eutic powder (Doful powder), opium alkaloid hydrochloride injection (Opial), opium alkaloid atropine injection (Opiato), and opium alkaloid scopolamine injection (Opisco, Pansco, Weak Pansco). In addition to injections, morphine hydrochloride is available in granules, tablets, liquid, and capsules for internal use, and suppositories are often used for external use. Diethylacetylmorphine hydrochloride (heroin) has the strongest analgesic effect, but it has serious side effects and is not used as a medicine. Its transportation is also restricted around the world.

(2) Synthetic narcotics include pethidine hydrochloride and fentanyl citrate, and combination drugs include "Petilorfan," "Weak Petilorfan," and "Talamonal."

Non-narcotic analgesics

Morphine substitutes include pentazocine, buprenorphine hydrochloride, butorphanol tartrate, tramadol hydrochloride, and eptazocine hydrobromide, all of which are used as injections, tablets, and suppositories to treat cancer and post-operative pain.

Other antipyretic, analgesic and anti-inflammatory drugs include the non-pyrine type, acetaminophen, and the pyrine type, sulpyrine.

The most commonly used painkillers are nonsteroidal anti-inflammatory drugs, which also have antipyretic and antithrombotic effects. These include aspirin, mefenamic acid (Pontal), loxoprofen (Loxonin), ibuprofen (Brufen), diclofenac sodium (Voltaren), and indomethacin.

鎮痛剤。俗にいう痛みどめのことで、中枢神経に作用して、意識を失うことなく疼痛(とうつう)を取り除き、または軽減する薬剤をいう。麻薬性鎮痛薬と非麻薬性鎮痛薬がある。麻薬性鎮痛薬にはアヘンを原料とする天然麻薬と、純化学的に合成した合成麻薬があり、非麻薬性鎮痛薬にはペンタゾシン、ブプレノルフィン塩酸塩などがあり、そのほかアセトアミノフェンをはじめとする解熱鎮痛薬および消炎鎮痛薬がある。

麻薬性鎮痛薬

(1)天然麻薬　鎮痛薬でもっとも有効なものはアヘンアルカロイド製剤の塩酸モルヒネであるといわれる。これは耽溺(たんでき)性、習慣性があり、禁断現象がみられることから、麻薬及び向精神薬取締法で規制されている。また、これの欠点は呼吸麻痺(こきゅうまひ)がみられることである。ほかにアヘンアルカロイド製剤としては、アヘン末、アヘン散、アヘン吐根散(とこんさん)（ドーフル散）、アヘンアルカロイド塩酸塩注射液（「オピアル」）、アヘンアルカロイド・アトロピン注射液（「オピアト」）、アヘンアルカロイド・スコポラミン注射液（「オピスコ」「パンスコ」「弱パンスコ」）などがある。塩酸モルヒネは注射液のほか、内用として顆粒(かりゅう)、錠剤、液剤、カプセルがあり、外用には坐薬(ざやく)が繁用される。塩酸ジエチルアセチルモルヒネ（ヘロイン）は鎮痛作用がもっとも強いが、副作用も大きいため医薬品としては使用されない。その持ち運びも世界中で規制されている。

(2)合成麻薬　ペチジン塩酸塩、フェンタニールクエン酸塩があり、配合剤に「ペチロルファン」「弱ペチロルファン」「タラモナール」がある。
