Allergen - Arerugen (English spelling)

A substance that triggers an allergic reaction. An allergy is a phenomenon in which the immune system, which is supposed to protect the body from foreign invaders, reacts to other substances that are supposed to be harmless, causing adverse symptoms for the body. This immune reaction occurs when an external antigen binds to an antibody produced in the body, and an allergen is the antigen that triggers this allergy. Allergens are usually proteins from organisms other than the body itself.

Which substances are allergens varies from person to person and over time. This depends on the antibodies produced by immune cells and on which cell surface those antibodies are located. There are two stages to finding out what the allergen is. First, confirm the situation that triggers the symptoms, i.e., confirm the facts of what substance is causing the symptoms and where it is coming into contact, and find possible allergens. Second, check with a blood or skin test to see if there is immunoglobulin E (IgE) antibody against that substance. If it is not linked to the symptoms, even if IgE antibodies are detected, it cannot be said that it is an allergen.

IgE antibody tests are usually performed by measuring which organisms the antibody is directed against, but in recent years, it has become possible to measure which organisms and substances the antibody is directed against. For example, in addition to testing for IgE antibodies against peanuts, it is now possible to test for the presence or absence of IgE antibodies against Ara h2, a peanut protein that often causes allergies.

In the past, allergies were considered to be immune reactions that were detrimental to the body, and allergens were considered to be the causative substances, so they were considered a negative presence that threatened the body. Therefore, the focus of allergy treatments has been on identifying and removing allergens. However, it cannot be said that allergens need to be removed unless they trigger an immune reaction. Therefore, in recent years, treatments have been attempted using allergen immunotherapy, which aims to create a state in which the presence of allergens does not lead to allergic symptoms.

アレルギー反応の誘因となる物質のこと。アレルギーは、自己を外敵から守る仕組みであるはずの免疫が、本来は無害なはずの他者に対して反応することにより、体にとって不利益な症状が引き起こされる現象である。この免疫反応は外部の抗原と、体内でつくられた抗体が結合することによりおこるが、アレルゲンはこのアレルギーのきっかけとなる抗原のことをさす。アレルゲンとなる物質は通常、自己以外の生物のタンパク質である。

　どの物質がアレルゲンとなっているかは、人により、また経過により異なる。それは免疫を担当する細胞がつくる抗体が異なることや、その抗体がどの細胞の表面にいるかなどの条件による。何がアレルゲンとなっているかをみつけるためには、二段階で確認を行う。第一に、症状のきっかけとなっている状況、すなわちどういった物質がどこに接触していることで症状が発現しているのか、事実を確認し、アレルゲンの候補をみつけだす。第二に、その物質に対する免疫グロブリンE（IgE）抗体があるかどうかを、血液検査や皮膚検査で確認する。症状と結び付いていない場合は、IgE抗体が検出されても、アレルゲンとなっているとはいえない。

　IgE抗体の検査は、通常どの生物に対する抗体かを測定することで行われているが、近年ではどの生物のどの物質に対する抗体かを測定することも行われるようになってきている。たとえば、ピーナッツに対するIgE抗体を検査する方法に加えて、ピーナッツのタンパク質のうち、とくにアレルギーの原因となることの多いタンパク質であるAra h2に対するIgE抗体の有無を検査することができるようになっている。

　かつてアレルギーは、免疫反応のうち生体にとって不利に働くものととらえられ、アレルゲンはその原因物質とされていたので、生体をおびやかす原因としての負の存在と位置づけられてきた。したがってアレルギーの対応は、アレルゲンを同定し、除去することに主眼が置かれてきた。しかしアレルゲンは、それがきっかけとなり免疫反応が引き起こされなければ、除去する必要があるとはいえない。そこで近年では、アレルゲン免疫療法によって、アレルゲンの存在がアレルギーの症状につながらない状態を目ざす治療が試みられている。
