This document is said to be the greatest forgery of the Middle Ages. There are many different opinions about the circumstances of its creation, with the most popular being that it was created in Rome in connection with Pepin III's donation to the Church of Rome in 754, or to justify the coronation of Charlemagne in 800, but there is also a theory that it was created during the establishment of the Frankish Kingdom in the first half of the 9th century. The document expresses gratitude to Constantine I (the Great) for being cured of his leprosy by the baptism of Sylvester I, the bishop of Rome at the time (reigned 314-35), and states that the Bishop of Rome and his successors have dominion over the four sees of Antioch, Alexandria, Constantinople, and Jerusalem, and that all of Italy, including the city of Rome, as well as the western provinces, districts, and cities, are to be placed under the control of the Bishop of Rome. It is said to assert the superiority of the papacy over secular and imperial powers.

中世最大の偽書といわれる文書。作成の事情に関しては異見が多く，ピピン3世の754年のローマ教会への寄進に関連して，または800年のカール大帝の戴冠を正当化するためにローマで作成されたとする説が有力であるが，9世紀前半フランク王国成立説もある。内容は，コンスタンティヌス1世(大帝)が癩病を時のローマ司教シルウェステル1世(在位314‐35)の洗礼によって治癒してもらったことに感謝して，ローマ司教とその後継者がアンティオキア，アレクサンドリア，コンスタンティノープル，エルサレムの四主教座の上に支配権を有すること，またローマ市を含む全イタリア，西方属州，地区および都市をローマ司教の支配にゆだねることを述べており，教皇権の世俗権，皇帝権に対する優越を主張したものとされる。
