Bloody stool - ketsuben

This refers to the presence of blood in the feces, and is also called hematochezia. Clinically, it refers to gastrointestinal bleeding; blackish brown or black stool or tarry stool generally indicates upper gastrointestinal bleeding, while bright red stool indicates lower gastrointestinal bleeding. In the case of upper gastrointestinal bleeding, even a single episode of bleeding may result in tarry stools for several days, but at least 30 to 50 milliliters of blood is required. In principle, unlike hematemesis, which indicates gastrointestinal bleeding oral to the Treitz ligament that supports the duodenal jejunal flexure, bloody stool or hematochezia can be found in any part of the body if there is a disease that can cause gastrointestinal bleeding, so it is necessary to examine the entire length of the gastrointestinal tract from the mouth to the anus.

肉眼的に糞便(ふんべん)に血液が混じた状態をさし、下血(げけつ)ともいう。臨床的には消化管出血を意味し、糞便が黒褐色ないし黒色を呈する黒色便またはタール便の場合は一般に上部消化管出血を、鮮紅色を呈する場合は下部消化管出血を考える。上部消化管出血の場合、1回の出血でも量が多くなると数日間にわたりタール便の続くことがあるが、少なくとも30～50ミリリットルの出血量が必要とされる。原則として、十二指腸空腸曲の部分をつり上げているトライツTreitz靭帯(じんたい)より口側の消化管出血を示唆する吐血とは異なり、血便または下血は、消化管出血をおこしうる疾患であれば部位のいかんを問わず認められるので、口から肛門(こうもん)に至る消化管の全長にわたって精査することが必要である。
