[What kind of disease is it?]
This is a disease in which the mucous membrane or surface of the skin becomes infected with bacteria, usually associated with phimosis ("phimosis").
In phimosis, the surface of the foreskin is divided into an outer plate and an inner plate by the foreskin ring, which is the tip under normal natural conditions, and the inner plate is in contact with the glans.
When bacteria grow between the inner foreskin and the glans, causing inflammation, it is called balanoposthitis.
Symptoms include redness, swelling, sores, and pain in the glans and foreskin, and sometimes ulcers and pus discharge, as well as pain when urinating. However, it is often not accompanied by high fever or abnormalities in urination itself.
[Cause]
When the glans is covered by the foreskin for a long period of time, smegma (also called smegma, a type of dirt specific to this area) accumulates between the inner plate of the foreskin and the glans. Smegma is easily contaminated by urine and is prone to bacterial infection. In addition, due to conditions such as body temperature and moisture from urine, and limited exposure to the outside air, it is thought to be a breeding ground for bacterial growth.
Therefore, although balanoposthitis is common in children, who often suffer from phimosis, it can also occur in adults who do not retract the foreskin (pulling the epidermis toward the base of the penis to expose the glans).
[Testing and diagnosis]
Diagnosis is relatively easy based on the symptoms and the foreskin. The type of causative bacteria can be identified by bacterial culture tests on the smegma and exudate, but in reality, such tests are often not performed.
[Treatment]
Antibiotics and chemotherapy drugs are administered. Depending on symptoms, anti-inflammatory and analgesic drugs are also used.
In the case of pseudo-phimosis, the foreskin is turned back and disinfected, and then an antibiotic topical ointment is applied. In mild cases, topical ointments alone may be sufficient.
[Precautions in daily life]
Be sure to keep the glans and foreskin clean by frequently retracting the foreskin to expose the inner foreskin and glans to the outside air and by avoiding touching the penis with dirty hands.
As a preventative measure, it is important to prevent the accumulation of smegma by flipping the foreskin back when bathing and washing it lightly with lukewarm water.
To prevent recurrence, surgery for phimosis is recommended. In particular, surgery is recommended for true phimosis or when recurrence is frequent.

[どんな病気か]
　ふつう、包茎（ほうけい）（「包茎」）にともなってみられる、粘膜（ねんまく）や皮膚の表面が細菌に感染する病気です。
　包茎では、包皮（ほうひ）の表面は、通常の自然な状態で先端部となる包皮輪（ほうひりん）を境にして、外板（がいばん）と内板（ないばん）に分けられ、内板は亀頭（きとう）に接しています。
　この包皮内板と亀頭の間に細菌が増殖して炎症がおこった状態を、亀頭包皮炎といいます。
　症状としては、亀頭および包皮の発赤（ほっせき）、腫（は）れ、ただれ、疼痛（とうつう）、ときには潰瘍（かいよう）ができたり膿（うみ）が出たりし、また排尿痛があることもあります。しかし、高熱や排尿そのものの異常はともなわないことが多いものです。
[原因]
　亀頭が包皮におおわれている状態が長期間におよぶと、包皮の内板と亀頭の間に恥垢（ちこう）（スメグマともいい、この部分特有の垢（あか））がたまってきます。恥垢は尿で汚染されやすく、細菌が感染しやすいうえ、体温や尿による湿り、外気にふれにくいといった条件から、細菌繁殖の温床となりやすいと考えられます。
　したがって亀頭包皮炎は、包茎であることが多い子どもに多くみられますが、包皮の翻転（ほんてん）（陰茎（いんけい）の根もとへ表皮をよせて亀頭を露出させる）を行なわない成人にも生じます。
[検査と診断]
　症状と亀頭包皮をみれば、診断は比較的容易です。恥垢や浸出液の細菌培養検査をすれば、原因菌の種類がわかりますが、実際には、こうした検査は行なわれないことが多いようです。
[治療]
　抗生物質や化学療法薬などの抗菌薬を服用します。症状によっては消炎鎮痛薬を併用します。
　仮性包茎では、包皮を翻転して消毒した後、抗生物質の外用剤を塗布します。軽症では外用剤のみで十分なことがあります。
[日常生活の注意]
　包皮の翻転をしばしば行ない、包皮内板と亀頭を外気に触れさせるようにしたり、汚れた手で陰茎を触らないようにするなど、亀頭と包皮をつねに清潔に保つように心がけます。
　予防としては、入浴時に包皮を翻転し、ぬるま湯で軽く洗うなどして、恥垢がたまらないようにすることがたいせつです。
　再発予防には、包茎を手術するのがよいでしょう。とくに真性包茎や再発をくり返す場合には、積極的に手術するのがよいと考えられます。
