Lumbago - Lumbago

Severe lower back pain that occurs when suddenly twisting the waist or trying to lift something heavy is commonly called "lumbago." Lower back movement is restricted due to the pain, and the pain worsens when moving the body. In severe cases, standing and walking can be difficult. There are various underlying diseases, but the most common are partial contusion of the psoas muscle or damage to the lumbar intervertebral disc. It is also said that there may be cases of rupture of the lumbar interspinous ligament. In either case, the pain will subside if you lie down and rest. With a partial contusion of the psoas muscle, there is localized tenderness in the paraspinal muscle on one side, and the pain is usually particularly severe when bending forward. In this case, the condition generally improves in a relatively short period of time, but it can become chronic. In cases of lumbar disc damage, disc herniation can occur, causing the spine to curve, making it difficult to move the lumbar spine, and causing radicular sciatica. In either case, it is necessary to see a specialist to clarify the cause of the pain and receive appropriate treatment.

急に腰をひねったり、重い物を持ち上げようとしたときなどに強い腰痛が発生するのを俗に「ぎっくり腰」とよんでいる。痛みのために腰部の運動が制限され、また体を動かすと腰痛が増強し、痛みが激しい場合は起立歩行が困難なこともある。その本体となる疾患はいろいろあるが、一般には腰筋の部分的挫傷(ざしょう)あるいは腰椎(ようつい)椎間板損傷であることが多い。また、腰椎棘間靭帯(きょくかんじんたい)の断裂の場合もあるとされている。いずれにしても、寝たまま安静にしていれば痛みは軽減する。腰筋の部分的挫傷では片側の傍脊柱(ぼうせきちゅう)腰筋に限局性の圧痛があり、普通は腰椎前屈時に痛みがとくに強くなる。この場合は一般に比較的短期間に軽快するが、慢性化することもある。腰椎椎間板損傷では椎間板ヘルニアの場合があり、脊柱が側彎(そくわん)し、腰椎の運動が困難で、根性坐骨(こんせいざこつ)神経痛をきたす。いずれの場合も専門医に受診して痛みの原因を明確にし、適切な治療を受けることが必要である。
