Lochia - lochia

This refers to the secretions from the genitals that are seen after childbirth. It is mainly bleeding from the part of the endometrium where the placenta has detached, secretions from other parts of the endometrium mixed with small amounts of secretions from the cervix and vagina. After childbirth, as the placenta is repaired, the color and amount of lochia changes. For about two weeks after childbirth, there is a considerable amount of bleeding, so it is menstrual-like bloody and is called red lochia. After that, as the amount of bleeding decreases, the color changes from dark brown to light brown, which is called brown lochia and lasts for about two weeks. Once the uterus and birth canal are repaired and bleeding has almost stopped, yellowish yellow lochia or whitish white lochia appears. By the time the lochia disappears, there are often days when light brown lochia is present and days when it is not, and it takes 4 to 6 weeks, but there is a large amount of individual variation. Lochia usually has a distinctive fishy or sweet-sour smell. If large blood clots are expelled, it is considered abnormal, and if there is a foul odor other than the characteristic lochia smell, an infection in the uterus or birth canal is suspected. Also, if part of the placenta or amniotic membrane remains in the uterus, the amount of red or brown lochia will be large and will last for a long time.

分娩(ぶんべん)後にみられる性器からの分泌物をいう。主として胎盤が剥離(はくり)した子宮内膜部からの出血と、他の子宮内膜面からの分泌液に少量の子宮頸管(けいかん)、腟(ちつ)からの分泌液が混じったものである。分娩後、この胎盤剥離面が修復されてくるとともに、悪露の色調と量が変化してくる。産後約2週間は出血量がかなりあるため月経様の血性で、赤色悪露とよばれる。その後、出血量が減少してくると暗褐色から淡褐色の色調に変化するが、これは褐色悪露とよばれ、約2週間持続する。子宮および産道が修復されて出血がほとんどなくなってくると、黄色調の黄色悪露や白色調の白色悪露がみられる。悪露が消失するころには、淡褐色の悪露がある日とない日があることが多く、結局4～6週間かかるが、個人差が大きい。悪露には特有な生臭い、あるいは甘酸っぱいにおいがあるのが普通である。なお、大きな凝血が排出される場合は異常と考えられ、悪露特有のにおい以外の悪臭がある場合は子宮内あるいは産道の感染が疑われる。また、胎盤や卵膜の一部が子宮内に残っていると、赤色悪露あるいは褐色悪露の量が多く、しかも長く持続する。
