Hormones are chemical substances secreted by the endocrine organs of the body to regulate the metabolism of the entire body, and hormone drugs are used for medicinal purposes. Hormones are not only those secreted by the body, but also synthetic chemical substances that have been developed to have hormone-like effects, and both natural and synthetic products are actually used. In addition, to treat secretory dysfunction caused by hormone deficiency or excess, hormone drugs are administered in cases of deficiency, and anti-hormonal drugs are administered in cases of excess. In addition to the inherent hormonal effects, some hormone drugs have their own pharmacological effects, just like regular drugs, and there are many cases in which these pharmacological effects are utilized. For example, cortisone, a hormone produced by the adrenal cortex, is used as an anti-inflammatory drug.

Hormones are classified into three categories: (1) by the tissues or organs they secrete, (2) by chemical classification, and (3) by physiological action. The chemical classification includes protein hormones, steroid hormones, and others. The protein category includes thyroid hormones, salivary gland hormones, pituitary hormones, and pancreatic hormones, while the steroid category includes sex hormones, anabolic hormones, and adrenal cortex hormones.

The first hormone obtained in crystalline form was adrenaline from the adrenal medulla, but adrenaline is known more as a drug that acts on the autonomic nervous system than as a hormonal drug. Early on, most hormone drugs were extracted from animal organs, urine, and blood, but the synthesis of steroid hormones such as sex hormones and adrenal cortex hormones became more common, and at the same time, substances with completely different chemical structures that had hormonal effects were discovered, and with the improvement of synthesis technology, polypeptide hormones also began to be synthesized. Below is a brief description of the main hormones used for medicinal purposes.

Pituitary hormones

There are six known anterior pituitary hormones: adrenocorticotropic hormone (ACTH), thyroid-stimulating hormone (TSH), growth hormone (GH), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and prolactin. Of these, TSH, GH, FSH, and LH are used as medicines. ACTH was used as an injectable corticotropin, but is no longer used at all due to the development of tetracosynthate acetate and zinc tetracosynthate acetate injections. TSH is used in the form of an injectable preparation for diagnosing pituitary and thyroid function, and FSH and LH are formulated as three types of gonadotropic hormones: serum, placental, and pituitary. GH is a specific medicine for pituitary dwarfism, and is extracted from the human brain and is also produced by genetic engineering.

Posterior pituitary hormones include oxytocin and vasopressin. Oxytocin is used for labor induction, mild labor pains, and atonic bleeding, while vasopressin is used to treat diabetes insipidus. Desmopressin acetate is effective as a nasal spray in treating central diabetes insipidus.

Salivary gland hormones

It is used to treat gastroptosis, osteoarthritis, and progressive muscular atrophy, and is commonly known as "Parotin."

Thyroid hormones

It is available in desiccated thyroid (powder/tablets), liothionine sodium (tablets), and levothyroxine sodium (tablets), and is used to treat hypothyroidism, pituitary myxedema, and simple goiter.

Parathyroid hormone

A protein preparation consisting of 84 amino acids extracted from bovine parathyroid glands was used to treat hypofunction or absence of the parathyroid glands, but its sale was discontinued due to side effects. In addition, a synthetic peptide preparation (teriparatide acetate) used in the differential diagnosis of hypoparathyroidism is commercially available.

Anabolic hormones

When it was discovered that male hormones have an anabolic effect, steroids with strong anabolic effects were synthesized one after another, including methenolone, methandrostenolone, nandrolone, oxaborone, porandiol, ethylnandrol, mestanolone, stanozolol, furazabol, and oxymetholone.

Adrenal Hormones

There are adrenal medulla hormones and adrenal cortex hormones. Adrenaline, an adrenal medulla hormone, has the official name epinephrine and is used as a sympathomimetic. Adrenal cortex hormones include mineralocorticoids and glucocorticoids. Mineralocorticoids include aldosterone, and glucocorticoids include cortisone and cortisol. Of these, it is the cortisone system that has made astonishing progress as a medicine. The body only produces cortisone and hydrocortisone (cortisol), but among these, steroids with high efficacy have been synthesized one after another with the aim of achieving anti-inflammatory effects. These include prednisolone, methylprednisolone, triamcinolone, dexamethasone, paramethasone, and betamethasone, and many adrenal cortex hormones that are used only as external agents have been developed. These include diflucortone, fluorometholone, beclomethasone, and fluocinolone.

Sex hormones

They are divided into male and female hormones. Male hormones such as testosterone and methyltestosterone are used for male genital dysfunction, while those with strong anabolic effects are used as anabolic hormones. Dromostanolone propionate is used to treat fibrocystic breast disease. Epitiostanol and mepitiostane, which have strong anti-estrogenic effects, are used for breast cancer.

Female hormones include follicular hormones and luteinizing hormones. Follicular hormones include estradiol, mestranol, and estriol. Synthetic follicular hormones that do not have a steroid nucleus include diethylstilbestrol and hexestrol, both of which are used to treat prostate cancer. Progesterone is the main component of luteinizing hormones and is formulated, but dimethisterone, norethisterone, allylestrenol, and other hormones with strong luteinizing hormone effects have also been synthesized and formulated.

In addition, these sex hormone drugs come in mixed hormone preparations, including those that combine estrogen and progesterone, and mixed preparations of male hormones and estrogen.

Other Hormones

Pregnanediol is used to treat acne vulgaris, cyproterone to treat precocious puberty, danazol to treat endometriosis, bromocriptine mesylate to treat acromegaly and pituitary gigantism, and calcitonin and elcatonin to treat hypercalcemia and osteo-Behçet's disease.

Pancreatic hormones include glucagon secreted from A cells in the islets of Langerhans, and insulin secreted from B cells. Insulin is an essential drug for the treatment of diabetes.

生体の内分泌器官より分泌され、全身の物質代謝を調節する化学物質をホルモンといい、これを医薬用としたものがホルモン剤である。ホルモン剤としては、生体から分泌されるものばかりでなく、合成した化学物質でホルモン様作用をもつものが開発され、実際には天然品および合成品が使用されている。また、ホルモンの欠乏または過剰によっておこる分泌機能障害に対し、欠乏による場合にホルモン剤が投与され、過剰による場合に対しては抗ホルモン剤が投与される。なお、ホルモン剤は本来のホルモン作用のほか、一般の薬物と同様に、それ自身の薬理作用をもつものがあり、その薬理作用を利用する例も多くなった。たとえば、副腎(ふくじん)皮質ホルモンであるコルチゾンは、むしろ抗炎症剤として用いられている。

　ホルモン剤の分類には(1)内分泌する組織や器官による分類、(2)化学的分類、(3)生理作用による分類があり、化学的分類としてはタンパク質系ホルモン、ステロイド系ホルモン、その他がある。タンパク質系には甲状腺(せん)ホルモン、唾液(だえき)腺ホルモン、下垂体ホルモン、膵臓(すいぞう)ホルモンなどが含まれ、ステロイド系には性ホルモン、タンパク同化ホルモン、副腎皮質ホルモンなどがある。

　結晶形として得られたホルモンは副腎髄質からのアドレナリンが最初であるが、アドレナリンはホルモン剤としてよりも、むしろ自律神経系に作用する薬物として知られる。ホルモン剤は、初期には動物の臓器をはじめ、尿や血液から抽出したものがほとんどであったが、性ホルモンや副腎皮質ホルモンなどステロイド系ホルモンの合成が盛んとなり、一方ではまったく化学構造の異なるものでホルモン作用のあるものが発見され、合成技術の向上とともにポリペプチド系のホルモンも合成されるようになった。以下、おもなホルモンについて医薬用に使われているものを簡単に述べる。

下垂体ホルモン

下垂体前葉ホルモンには副腎皮質刺激ホルモン（ACTH）、甲状腺刺激ホルモン（TSH）、成長ホルモン（GH）、卵胞刺激ホルモン（FSH）、黄体形成ホルモン（LH）、乳腺刺激ホルモン（プロラクチン）の6種が知られ、このうちでTSH、GH、FSHおよびLHが医薬品として使用されている。ACTHは、注射用コルチコトロピンとして用いられたが、酢酸テトラコサクチドおよび酢酸テトラコサクチド亜鉛注射液の開発により、現在ではまったく用いられていない。TSHは注射用の製剤が下垂体および甲状腺の機能診断に用いられており、FSHおよびLHでは血清性、胎盤性、下垂体性の3種の性腺刺激ホルモンが製剤化されている。GHは下垂体性低身長症の特効薬で、ヒトの脳から抽出されており、遺伝子工学により製造したものもある。

　下垂体後葉ホルモンにはオキシトシンとバソプレッシンがあり、オキシトシンは分娩(ぶんべん)誘発、微弱陣痛、弛緩(しかん)出血に、バソプレッシンは尿崩症の治療にそれぞれ用いられる。なお、酢酸デスモプレシンは点鼻剤として中枢性尿崩症の治療に有効である。

唾液腺ホルモン

胃下垂、変形性関節炎、進行性筋萎縮症(しんこうせいきんいしゅくしょう)などに用いられ、一般に「パロチン」の名で知られる。

甲状腺ホルモン

乾燥甲状腺（末・錠）、リオチオニンナトリウム（錠）、レボチロキシンナトリウム（錠）があり、甲状腺機能低下症、下垂体性粘液水腫(すいしゅ)、単純性甲状腺腫などに用いられる。

上皮小体ホルモン

ウシの上皮小体（副甲状腺）から抽出したアミノ酸84個からなるタンパク質製剤が上皮小体の機能低下または欠損に用いられたが、副作用のため発売が中止された。また、副甲状腺機能低下症の鑑別診断に用いられる合成ペプチド製剤（酢酸テリパラチド）が市販されている。

タンパク同化ホルモン

男性ホルモンにタンパク同化作用のあることがわかり、同作用の強いステロイドが次々と合成された。メテノロン、メタンドロステノロン、ナンドロロン、オキサボロン、ポランジオール、エチルナンドロール、メスタノロン、スタノゾロール、フラザボール、オキシメトロンなどがある。

副腎ホルモン

副腎髄質ホルモンと副腎皮質ホルモンがある。副腎髄質ホルモンのアドレナリンは局方名エピネフリンで、交感神経興奮剤として用いられる。副腎皮質ホルモンには鉱質コルチコイドと糖質コルチコイドがあり、鉱質コルチコイドにはアルドステロンが、糖質コルチコイドにはコルチゾンおよびコルチゾールがある。このうち、医薬品として驚異的発展を遂げたのはコルチゾン系である。生体内ではコルチゾン、ヒドロコルチゾン（コルチゾール）のみを産生するが、その作用のうち、抗炎症作用を目標として効力の大きいステロイドが次々と合成された。プレドニゾロン、メチルプレドニゾロン、トリアムシノロン、デキサメタゾン、パラメタゾン、ベタメタゾンがあり、さらに外用剤としてのみ使用される副腎皮質ホルモン剤が多く開発された。ジフルコルトン、フルオロメトロン、ベクロメタゾン、フルオシノロンなどがある。

性ホルモン

男性ホルモンと女性ホルモンに分けられる。男性ホルモン剤としてはテストステロン、メチルテストステロンが男性性器機能不全に用いられるほか、タンパク同化作用の強いものはタンパク同化ホルモンとして使用される。プロピオン酸ドロモスタノロンは乳腺症の治療に用いられる。抗エストロゲン作用の強いエピチオスタノールやメピチオスタンは乳癌(にゅうがん)に用いられる。

　女性ホルモンには卵胞ホルモンと黄体ホルモンがある。卵胞ホルモンにはエストラジオール、メストラノール、エストリオールがある。また、ステロイド核をもたない合成卵胞ホルモンにはジエチルスチルベストロールとヘキセストロールがあり、ともに前立腺癌の治療に応用されている。黄体ホルモンはプロゲステロンが本体であり、製剤化されているが、やはり黄体ホルモン作用の強いジメチステロン、ノルエチステロン、アリルエストレノールなどが合成され、製剤化された。

　なお、これら性ホルモン剤は混合ホルモン剤として、卵胞ホルモンと黄体ホルモンを配合したもの、男性ホルモンと卵胞ホルモンの混合製剤がある。

その他のホルモン

プレグナンジオールは尋常性痤瘡(ざそう)に用いられ、シプロテロンは思春期早発症に、ダナゾールは子宮内膜症治療剤として用いられる。また、メシル酸プロモクリプチンは末端肥大症や下垂体性巨人症の治療に用いられ、カルシトニンやエルカトニンは高カルシウム血症や骨ベーチェット病に用いられる。

　なお、膵臓ホルモンにはランゲルハンス島のA細胞から分泌されるグルカゴンとB細胞から分泌されるインスリンがある。インスリンは糖尿病の治療になくてはならない薬剤である。
