Check for secondary sexual characteristics and male sexual dysfunction

The male hormone testosterone is secreted from the testicles and plays a role in the expression of secondary male sexual characteristics, maintaining reproductive function, and promoting protein anabolism.

It is also present in small amounts in women, where it is low from birth until puberty, reaches adult levels during puberty, and then shows a slight decline after menopause.

In men, this hormone is measured to investigate the deficiency of secondary sexual characteristics and the decline of male sexual function in adults.

Measures based on test results

Because testosterone levels fluctuate greatly throughout the day, the test is either repeated multiple times during a fixed period in the morning while the man is resting, or measured several times on the same day at different times, and the average value of these is then determined comprehensively.

The main diseases suspected are

◆ Men: High levels → Leydig cell tumors, precocious puberty

Low value → congenital azoospermia, Klinefelter syndrome, undescended testicles

◆Women: High levels → Virilizing ovarian tumors, Cyst News Ovarian syndrome, virilizing adrenal tumors, idiopathic hirsutism

Low level → hypopituitarism, isolated LH deficiency

　男性ホルモンのテストステロンは睾丸から分泌されるホルモンで、男性の第二次性徴の発現、生殖機能維持、蛋白同化作用促進などの役割を果たしています。

　女性にもわずかに存在し、生後から思春期までは低値で、思春期に成人レベルに達したのち、閉経後には若干、低下する変動を示します。

　このホルモンは男性の場合、第二次性徴の不全や、成人での男性機能低下などを調べるとき測定します。

検査値からの対策

　テストステロンは日内変動が大きいため、午前中の一定時間の安静にした状態で、複数回検査を繰り返すか、あるいは同じ日に時間をずらして数回測定して、これらの平均値を総合的に判定します。

疑われるおもな病気などは

◆男性：高値→レイディッヒ細胞腫瘍、性早熟症

　　　　低値→先天性無精子症、クラインフェルター症候群、停留睾丸

◆女性：高値→男性化卵巣腫瘍、多嚢胞のうほう性卵巣症候群、男性化副腎腫瘍、特発性多毛症

　　　　低値→間脳下垂体機能低下症、LH単独欠損症
