This term has long been used in the past to describe children who are thin, generally considered to be weak, and have swollen lymph nodes in the neck, but it has no clear medical meaning. In Western medical terms, it is known as "ruireki (scab)." Ruireki refers to swelling of the cervical lymph nodes of children, especially those with a thymic lymphatic constitution, due to tuberculous lesions, and refers to a peculiar facial appearance accompanied by lesions such as eczema on the skin, small nodules on the cornea of ​​the eyes, or phlyctenular inflammation of the lips and nasal mucosa.

体格が細く、一般に弱い体とみなされ、頸(けい)部のリンパ節が腫(は)れているような小児に対して、昔から俗に用いられていたことばであるが、医学的には明らかな概念をもっていない。欧米の医学用語としては、いわゆる「るいれき（瘰癧）」がこれにあたる。るいれきとは、小児、とくに胸腺リンパ体質の子供の頸部リンパ節が結核性病変のために腫脹(しゅちょう)したものをさし、皮膚の湿疹(しっしん)、目の角膜におこる小結節すなわちフリクテン、口唇および鼻粘膜の炎症などの病変を伴って特異な顔つきになることを意味している。
