病原微生物を含んだ排出物や分泌物が,なんらかの原因で便池や下水から飲料に供する河川水や井戸水などに混入し,その水を介し,経口的に感染したり,水中の病原微生物が皮膚を通して感染する病気を水系伝染病という。上水道が完備しなかった時代には,河川水を飲用に供し,上流にたとえば赤痢患者が発生すると,その河川に沿って下流に赤痢患者がしばしば発生した。腸チフス,パラチフス,細菌性赤痢,アメーバ赤痢,ポリオなどが水を介し経口的に感染する。
