Hymen - shojomaku (English spelling) hymen

A membrane-like tissue at the boundary between the vaginal vestibule and the vagina of a virgin woman, narrowing the vaginal opening. The anterior part has the properties of skin, and the posterior part of the mucous membrane. Its shape and hardness vary from person to person. Physiologically, the hymen is circular, with the vaginal opening in the center; semilunar, with the posterior edge of the hymen higher than the anterior edge; split hymen, which is completely bisected vertically; semi-split hymen, which is partially bisected; cribriform hymen, with the vaginal opening with many small holes; and sickle hymen, which is wide in the center. In most cases, the hymen is broken during the first sexual intercourse, but it can also be broken for other reasons, and the hymen can remain intact even after sexual intercourse. Therefore, caution must be exercised in determining virginity from the condition of the hymen.

In cases of thickened or stenotic hymen, sexual intercourse becomes impossible due to pain, so hymenotomy is performed. After the hymen is torn during the first birth, it becomes deformed and vestigial, which is called a hymen scar. When there is no hole in the hymen and the vaginal opening is closed, this is called imperforate hymen. In this case, there is no menarche (pseudo-amenorrhea) because there is no route for menstrual blood to flow out, and blood accumulates in the body, causing repeated, transient, cyclical lower abdominal pain at the time of menstruation, which gets worse with each passing period. When these symptoms are present after delayed menarche, blood may have accumulated in the vagina (hematoma), the uterus (hematoma), or the fallopian tubes (hematoma), so by incising the hymen, regular menstruation can be achieved.

性交経験のない女性、すなわち処女の腟前庭(ちつぜんてい)と腟との境界にある膜状組織で、腟口はこれにより狭くなる。前面は皮膚、後方は粘膜の性質をもつ。形態とその硬度には個人差があってさまざまである。腟口が中央に位置する環状処女膜、処女膜の後縁が前縁より高くなっている半月状処女膜、垂直に完全に両分されている分離処女膜、部分的に両分されている半分離処女膜、腟口が多数の小孔となっている篩(ふるい)状処女膜、中央部が広がった鎌(かま)状処女膜などが生理的にみられる。多くの場合、初回の性交によって破れるが、それ以外の原因で破れることもあり、性交後も膜が保存される場合もある。したがって、処女膜の状態から処女性を決めることには慎重でなければならない。

　肥厚処女膜や処女膜強靭(きょうじん)症では痛みのために性交が不可能になるので、処女膜切開が行われる。処女膜は初回の分娩(ぶんべん)によって破れたのちは変形し痕跡(こんせき)的になり、処女膜痕とよばれる。処女膜に孔(あな)がなく腟口が閉じているのは処女膜閉鎖症である。この場合には月経血の流出路がないために初経がなく（偽無月経）血液は体内に貯留するので、月経の時期になると一過性の周期的な下腹痛が繰り返されて、回を重ねるごとに痛みはだんだん強くなる。初経が遅れてこのような症状がある場合には、腟に血液がたまる腟留血腫(けっしゅ)、子宮にたまる子宮留血腫、卵管にたまる卵管留血腫が考えられるので、処女膜を切開することにより順調な月経がみられるようになる。
