What is the disease?

This is a disease in which the maturation of secondary sexual characteristics due to the secretion of male and female hormones occurs at an early age.

In girls, puberty begins when the breasts begin to swell even slightly, but if breast development occurs before the age of 7 years and 6 months, there is a high possibility of precocious puberty. When only breast development is observed, it is necessary to distinguish this from premature breast development syndrome, which is thought to be caused by the temporary secretion of female hormones.

For boys, Testis ( Production ) ( Testicles ( Struggling ) ) reaches a size of 4ml or more, which marks the onset of puberty; if this occurs before the age of 9, there is a very high possibility of precocious puberty.

What is the cause?

How symptoms manifest

The early secretion of sex hormones causes a growth spurt. As maturation progresses, pubic hair begins to grow, the voice changes in boys, and menstruation begins in girls. If the cause is a brain tumor, the tumor may cause pressure and cause headaches. Tunnel vision ( Shakyosaku ) etc. may occur.

If left untreated, bone aging accelerates. Epiphysis ( Kotsutan ) These fuse prematurely, ultimately resulting in short stature and cessation of growth.

Testing and diagnosis

In the case of central precocious puberty, the basal levels of gonadotropic hormones and sex hormones increase, and LHRH ( Corpus luteum ( Song ) The gonadotropin-releasing hormone (GHRH) test shows pubertal levels of gonadotropin, and bone age and growth rate are accelerated.

In cases of peripheral precocious puberty, there is an increase in sex hormones, but the secretion of gonadotropic hormones is suppressed.

Precocious puberty occurring before age 2 is often diagnosed as a hamartoma or McCune-Albright syndrome. Tumors require imaging with CT or MRI.

Due to the transient secretion of female hormones, breast development (premature breast development) is often seen around the age of 1 year or 6 to 7 years, and it is necessary to distinguish it from precocious puberty, but usually there is no promotion of growth rate or bone age, and no increase in gonadotropic hormones. In most cases, the breasts will shrink naturally over time, but it may recur several times. In this case, autonomous ovarian cysts must be considered. Furthermore, many autonomous ovarian cysts progress to precocious puberty, so caution is required.

Treatment methods

In the case of peripheral precocious puberty, the first priority is to treat the tumor causing the condition. Hamartomas are indicative of central precocious puberty, but in most cases surgical removal is not required.

For central precocious puberty, the secretion of gonadotropic hormones is selectively suppressed with a drug called LHRH analog. It is administered subcutaneously once a month. In many cases, it shows a remarkable effect, with the progression of secondary sexual characteristics halting or regressing, and the progression of bone age slowing down.

In cases of true precocious puberty that are idiopathic or inoperable, gonadotropin-releasing hormone (GnRH) derivatives, which have a gonadotropin secretion suppressing effect, are administered to stop menstruation and prevent the progression of bone age.

　男性ホルモン、女性ホルモンの分泌による二次性徴の成熟が、早い年齢で起こってしまう病気です。

　女子では、乳房が少しでもふくらんできた時が思春期の開始ですが、乳房の発育が7歳6カ月以前に起こってしまった時、思春期早発症の可能性が高いといえます。乳房発育だけがみられるときは、一時的な女性ホルモンの分泌によると考えられる乳房早期発育症との区別が必要です。

　男子では、精巣(せいそう)（睾丸(こうがん)）が4ml以上の大きさになった時が思春期の開始ですが、それが9歳未満で起こった時、思春期早発症の可能性が非常に高いといえます。

原因は何か

　中枢性(ちゅうすうせい)と末梢性(まっしょうせい)に分類され、中枢性は通常の思春期の時のように下垂体(かすいたい)から性腺刺激ホルモンが分泌され、それにより性腺から性ホルモンが分泌されて起こります。女子に起こるものの多くは原因不明の特発性と呼ばれるものですが、男子に起こるものは脳腫瘍（胚芽腫(はいがしゅ)、過誤腫(かごしゅ)等）などの器質性の原因が多くみられます。

　末梢性の場合は、性腺または副腎(ふくじん)で性ホルモンがつくられて、思春期早発症が起こります。副腎腫瘍、卵巣腫瘍、治療不十分な先天性副腎皮質過形成症(ふくじんひしつかけいせいしょう)や特殊な遺伝子異常によるマックキューン・オルブライト症候群、家族性男性性早熟症(せいそうじゅくしょう)などがその原因です。

症状の現れ方

　性ホルモンが早期に分泌されることにより、成長のスパート（急激な進行）が起こります。成熟が進むと、陰毛の発育、男子では声変わり、女子では月経が認められます。原因が脳腫瘍による場合は、腫瘍の圧迫症状による頭痛・視野狭窄(しやきょうさく)などが起こることがあります。

　未治療で放置すると、骨年齢が促進して骨端(こつたん)が早期に融合するため、最終的に低身長で成長が終わります。

検査と診断

　中枢性思春期早発症の場合は、性腺刺激ホルモンと性ホルモンの基礎値の上昇とともに、LHRH（黄体(おうたい)形成ホルモン放出ホルモン）テストで性腺刺激ホルモンの思春期レベルの反応が認められます。また、骨年齢が促進し、成長率も高くなります。

　末梢性思春期早発症の場合は、性ホルモンの上昇は認められますが、性腺刺激ホルモンの分泌は抑制されています。

　2歳以前に発症する思春期早発症は、過誤腫またはマックキューン・オルブライト症候群によるものが多く診断されます。腫瘍の場合は、CTまたはMRIによる画像診断を必要とします。

　一過性の女性ホルモンの分泌により、乳房発育（乳房早期発育症）は1歳前後や6～7歳にみられることが多く、思春期早発症との区別を必要としますが、通常は成長率の促進や骨年齢の促進は認められず、性腺刺激ホルモンの上昇も認められません。経過をみていると、自然に乳房が退縮していくものがほとんどですが、何回も繰り返すこともあります。この場合は、自律性卵巣嚢腫(のうしゅ)を考える必要があります。また、自律性卵巣嚢腫の多くは、思春期早発症に移行していくので、注意が必要です。

治療の方法

　末梢性思春期早発症の場合は、原因の腫瘍などに対する治療が第一です。過誤腫は中枢性思春期早発症を示しますが、多くの場合は摘出手術を必要としません。

　中枢性思春期早発症は、LHRHアナログという薬剤で選択的に性腺刺激ホルモンの分泌を抑えます。月1回の皮下注射です。多くの場合、著しい効果を示し、二次性徴の進行停止・退縮がみられ、骨年齢の進行が緩やかになります。

病気に気づいたらどうする

　低年齢で乳房が大きくなってきた場合や、急に背が伸びてきた場合には、小児内分泌専門医に診てもらってください。

どんな病気か

　思春期早発症とは、年齢に比べて早期に第二次性徴が認められる病気です。男児では9歳未満で精巣、陰茎(いんけい)や陰嚢(いんのう)などの発育、10歳未満で陰毛の発生、11歳未満で腋毛(えきもう)、ひげの発生や声変わりが認められた場合、女児では7歳未満で乳房の発育、8歳未満で陰毛の発生、外陰部の早熟や腋毛の発生、9歳未満で初経が認められた場合が基準になります。

原因は何か

　下垂体性(かすいたいせい)ゴナドトロピンの分泌亢進(こうしん)が原因であるものを、真性(しんせい)思春期早発症と呼びます。原因不明の特発性のものが最も多いのですが、視床下部(ししょうかぶ)後部の腫瘍(しゅよう)（とくに過誤腫(かごしゅ)）による思春期発現抑制機構の障害によって発症することがあります。また、脳・髄膜炎(ずいまくえん)、水頭症(すいとうしょう)や頭部外傷などのさまざまな脳疾患が原因になる場合もあります。

　一方で、下垂体性ゴナドトロピンとは関連がなく、腫瘍などから分泌される性ホルモンなどの上昇によって発症するものを、仮性(かせい)思春期早発症と呼びます。原因としては精巣、卵巣や副腎の性ホルモン産生腫瘍、先天性副腎過形成(せんてんせいふくじんかけいせい)や、頭蓋内胚細胞腫(ずがいないはいさいぼうしゅ)などからのヒト絨毛性(じゅうもうせい)ゴナドトロピン（hCG）の過剰産生などがあげられます。

症状の現れ方

　男児では精巣の増大、陰茎や恥毛の発育がみられ、女児では乳房の腫大(しゅだい)、恥毛の発育や初経が現れます。第二次性徴の進行に伴って身長、体重や骨年齢の伸びも著しいのですが、早期に骨端線(こったんせん)が閉じるために最終的には低身長になってしまいます。

検査と診断

　骨年齢の測定やホルモン検査、画像検査による腫瘍の検索が必要になります。

治療の方法

　真性思春期早発症のなかで、器質的な脳疾患が原因の場合や仮性思春期早発症の場合は、腫瘍摘出などの原疾患の治療が優先されます。真性思春期早発症のなかで、特発性や手術不能な場合は、ゴナドトロピン分泌抑制作用のあるゴナドトロピン放出ホルモン（GnRH）誘導体を投与することで、月経の停止と骨年齢の進行防止を図ります。
