Sanno refers to Hiyoshi Taisha Shrine in Omi. The Omiya (Daihiei, Shaka Nyorai in its original form), Ninomiya (Kohiei, Yakushi Nyorai in its original form) are called Sanno Sansho or Sansei, with the Sanno Ryojin and Seishinshi (Amida Nyorai in its original form). The Hachioji (Thousand-Armed Kannon in its original form), Kyakunin (Eleven-Headed Kannon in its original form), Juzenshi (Jizo Bodhisattva in its original form), and Sannomiya (Samantabhadra in its original form) are added to make the Upper Seven Shrines. The number of shrines gradually increased with the addition of the Middle Seven Shrines and the Lower Seven Shrines, and they were called the "Sanno Twenty-One Shrines". The title Sanno appears in the "Commentary on the Twenty-Two Shrines" as "The matter of the title Sanno, first revered in the 10th year of the Kōnin era of the 52nd Emperor Saga." 山王とは近江の日吉(ひよし)大社のこと。大宮(大比叡,本地釈迦如来),二宮(小比叡,本地薬師如来)を山王両明神,聖真子(本地阿弥陀如来)を加えて山王三所または三聖といい,さらに八王子(本地千手観音),客人(本地十一面観音),十禅師(本地地蔵菩薩),三宮(本地普賢菩薩)を加えて上七社となし,さらに本地垂迹(ほんじすいじやく)説によってしだいに数を増し中七社,下七社を加えて〈山王二十一社〉と称した。山王の号は《二十二社註式》に〈山王号之事,第五十二代嵯峨天皇弘仁十年,始崇敬之〉と見える。
