Antihistamines - Antihistaminics

A drug that specifically antagonizes histamine. Histamine is one of the causes of allergies, and as it antagonizes this histamine, it is used as an antiallergic drug. That is, it is used to treat acute urticaria, as well as allergic dermatitis, allergic rhinitis, colds, and other upper respiratory tract infections such as runny nose, sneezing, and coughing. Many of them cause drowsiness as a side effect, and those with a strong effect are used as sleeping pills. Diphenhydramine is a typical drug, and drugs with stronger effects and fewer side effects have been developed.

Since allergies are caused not only by histamine but also by serotonin, drugs with anti-serotonin effects, sedative effects on the central nervous system, antiemetic, antitussive, and anti-Parkinson's effects have been developed, and the antihistamine effect has come to be considered as a part of them. There have also been cases where injections of antihistamines that suppress allergies have caused shock, so their use by injection has become extremely rare. Currently, they are used as over-the-counter drugs mixed into cold medicines, and drugs with few side effects such as drowsiness are used to treat allergic skin diseases and allergic rhinitis. Dimenhydrinate is also often used as a medicine for motion sickness.

The main ones include diphenhydramine hydrochloride, diphenhydramine tannate, chlorpheniramine maleate, clemizole hydrochloride, triprolidine hydrochloride, isothipendyl hydrochloride, promethazine hydrochloride, metrazine hydrochloride, alimemazine tartrate, diphenylpyraline hydrochloride, iproheptine hydrochloride, cycloheptazine hydrochloride (also used for weight gain), homochlorcyclizine hydrochloride, clemastine fumarate, carbinoxamine maleate, dimethindene maleate, mebhydroline napadisilate, and metaxine, and they also have effects other than antihistamine effects, such as antiacetylcholine and antibradykinin effects.

ヒスタミンと特異的に拮抗(きっこう)する薬物。アレルギーの一つの原因としてヒスタミンがあり、このヒスタミンと拮抗することから抗アレルギー剤として用いられる。すなわち、急性じんま疹(しん)をはじめとしてアレルギー性皮膚炎やアレルギー性鼻炎、感冒など上気道炎に伴う鼻汁、くしゃみ、咳嗽(がいそう)（咳(せき)）の治療に用いられる。副作用として眠気をもつものが多く、その作用の大なるものは睡眠薬として用いられる。ジフェンヒドラミンが代表的な薬剤であり、これより作用が大きく副作用の少ないものが開発された。

　アレルギーの原因としてはヒスタミンのみではなくセロトニンも関係することから、抗セロトニン作用のあるものや中枢神経に対する作用として静穏作用をもったもの、鎮吐作用、鎮咳作用や抗パーキンソン作用をもったものも開発され、抗ヒスタミン作用はその一部であるかのように考えられるようになった。またアレルギーを抑制する抗ヒスタミン薬が注射によりショックをおこした例もみられ、注射での応用はきわめて少なくなった。現在、かぜ薬の中に配合されて一般用医薬品として使用されているほか、眠気などの副作用の少ない薬剤がアレルギー性皮膚疾患やアレルギー性鼻炎の治療に用いられている。また乗り物酔いの薬としてもジメンヒドリナートがよく用いられる。

　おもなものをあげると、塩酸ジフェンヒドラミン、タンニン酸ジフェンヒドラミン、マレイン酸クロルフェニラミン、塩酸クレミゾール、塩酸トリプロリジン、塩酸イソチペンジル、塩酸プロメタジン、塩酸メトラジン、酒石酸アリメマジン、塩酸ジフェニルピラリン、塩酸イプロヘプチン、塩酸シクロヘプタジン（体重増加の目的でも使用される）、塩酸ホモクロルシクリジン、フマル酸クレマスチン、マレイン酸カルビノキサミン、マレイン酸ジメチンデン、ナパジシル酸メブヒドロリン、メタキシンなどがあり、抗アセチルコリン作用、抗ブラジキニン作用など抗ヒスタミン作用以外の作用もみられる。
