Anthelmintics. These are drugs used to eliminate parasitic worms, such as roundworms, pinworms, whipworms, hookworms, tapeworms, Schistosoma japonicum, pulmonary flukes, liver flukes, and other worms that live in the body. They are generally called vermicomycin. There are two types: those that kill the parasites, and those that paralyze the worms to promote their expulsion. The type of drug used varies depending on the type of parasite and the part of the body that is infected, but drugs that have a high affinity for the worms and a low affinity for the living body are desirable. In particular, drugs that are difficult to absorb from the intestinal tract are desirable for eliminating parasites that live in the intestinal tract. Anthelmintics are generally taken on an empty stomach, and a laxative is then taken later to expel the parasites along with the intestinal contents, but some drugs do not require a laxative. Recently, living conditions have improved, the incidence of parasites has decreased, and the use of anthelmintics has decreased.

Deworming agents include:

(1) Santonin: A typical anthelmintic used to exterminate roundworms and pinworms. It paralyzes the worms, which are then expelled by intestinal peristalsis. It is used in combination with a laxative. It is not used in the United States due to side effects such as yellow vision and liver damage. It is the active ingredient in the seed-shaped flowerheads of Chinese Artemisia anguicidae (commonly known as semen cina), Artemisia chinensis, Artemisia arbutifolia, and Artemisia cranberry, which belong to the Asteraceae family. It is a tasteless, odorless, white crystalline powder, and has also been successfully synthesized. In Japan, it was formerly known as semen or semen-en, and was used alone or in combination with kainic acid to exterminate roundworms.

(2) Kainic acid: The active ingredient in Kaijinsou (Makuri) and used to eliminate roundworms. It has fewer side effects than Santonin. It is also known by names such as Makunin and Jigenin.

(3) Piperazines The main ones are piperazine phosphate and diethylcarbamazine citrate. Piperazine phosphate is used to eliminate pinworms and roundworms. It has few side effects. Adults should take 2 to 3.5 grams once a day for 2 to 7 days. Diethylcarbamazine citrate is a specific medicine for filariasis, and the formulation is sold commercially under the name "Spatonin."

(4) Pyrantel pamoate: Effective against roundworms, pinworms, and hookworms. Administer 10 milligrams per kilogram of body weight once. Trade name: Comvantrin.

(5) Pyrvinium pamoate: Used to eliminate pinworms, roundworms, and hookworms. 2 to 5 milligrams per kilogram of body weight is administered at a time. The red pigment turns the stool red. Trade name: Pokir.

(6) Bithionol: Used to eradicate lung flukes, Paragonimus yokogawai, and Clonorchis liver flukes. 30 to 50 milligrams per kilogram of body weight is administered every other day. The commercial name is "Bithionol Tablets."

(7) Antimonyl Sodium Tartrate It is used as an injection to eradicate Schistosoma japonicum and Schistosoma hepatica. When sodium is replaced with potassium, it is called emetic tartrate.

(8) Others: Kamara and pomegranate peel are used as decoctions to exterminate tapeworms. Menma root (filmaron oil), chenopodium oil (active ingredient ascaridol), tetrachloroethylene, hexylresorcin, etc. are not used in Japan due to their strong side effects. In the United States, enteric preparations of hexylresorcin are used to exterminate roundworms, hookworms, pinworms, whipworms, etc., and these have insecticidal properties.

駆虫剤。体内に寄生する回虫、蟯虫(ぎょうちゅう)、鞭虫(べんちゅう)、十二指腸虫（鉤虫(こうちゅう)）、条虫、日本住血吸虫、肺ジストマ、肝ジストマ、肝蛭(かんてつ)などの蠕虫(ぜんちゅう)類を駆除する薬剤で、一般に虫下しともいわれる。寄生虫を殺滅する作用を有するものと、虫体を麻痺(まひ)させて排出を促す作用を有するものとの2種がある。寄生虫の種類と寄生部位によって用いられる薬剤の種類も異なるが、虫体に対する親和性が大で、生体に対する親和性の少ないものが望まれる。とくに腸管内に寄生する寄生虫の駆除には腸管から吸収されにくいものが望まれる。駆虫剤は一般的には空腹時に服用し、のちに寄生虫を腸内容物とともに排出させるため緩下剤を服用するが、下剤を必要としないものもある。最近は生活環境がよくなり、寄生虫の発生が少なくなって、駆虫剤の使用も減少してきた。

　駆虫剤には次のようなものがある。

(1)サントニン　代表的な駆虫剤で回虫や蟯虫などの駆虫に用いる。虫体を麻痺させて腸管の蠕動で排出される。下剤を併用する。黄視、肝障害など副作用があるため、アメリカでは使用されていない。キク科のシナヨモギ、ミブヨモギ、クラムヨモギの種子状をした小花頭のシナ花（通称セメンシナsemen cina）の有効成分である。無味・無臭の白色の結晶性粉末で、合成にも成功している。日本では古くはセメン、セメン円とよばれていたことがあり、単独またはカイニン酸との合剤として回虫の駆除に用いられた。

(2)カイニン酸　カイジンソウ（マクリ）の有効成分で回虫の駆除に用いられる。副作用はサントニンより少ない。マクニン、ジゲニンなどの名称がある。

(3)ピペラジン系　おもなものにリン酸ピペラジンとクエン酸ジエチルカルバマジンがある。リン酸ピペラジンは蟯虫、回虫の駆除に用いる。副作用は少ない。成人では1日1回2～3.5グラムを服用し、2～7日間連用する。クエン酸ジエチルカルバマジンはフィラリア症の特効薬で、製剤は「スパトニン」という名で市販されている。

(4)パモ酸ピランテル　回虫、蟯虫、ズビニ鉤虫に有効で、体重1キログラム当り10ミリグラムを1回投与する。市販名「コンバントリン」。

(5)パモ酸ピルビニウム　蟯虫、回虫、鉤虫の駆除に用いる。体重1キログラム当り2～5ミリグラムを1回投与する。赤い色素のため便が赤くなる。市販名「ポキール」。

(6)ビチオノール　肺吸虫、横川吸虫、肝吸虫の駆除に用いる。体重1キログラム当り30～50ミリグラムを隔日に投与する。市販名「ビチン錠」。

(7)酒石酸アンチモニルナトリウム　日本住血吸虫、肝ジストマの駆除に注射液として用いられる。ナトリウムをカリウムにかえたものを吐酒石という。

(8)その他　カマラ、ザクロ皮は条虫の駆除に煎剤(せんざい)として用いられる。メンマ根（フィルマロン油）、ヘノポジ油（有効成分アスカリドール）、四塩化エチレン、ヘキシルレゾルシンなど副作用が強いため、日本では用いられていない。アメリカではヘキシルレゾルシンの腸溶製剤が回虫、鉤虫、蟯虫、鞭虫などの駆除に用いられているが、これは殺虫的に作用する。
