Shoulder-hand syndrome

In addition to dull shoulder pain, movement is restricted, and as the condition progresses, external rotation is impaired. The pain extends from the neck to the arm, the hand becomes red and swollen, and the fingers cannot be fully extended or flexed. The condition is characterized by pain and a burning sensation in the peripheral limbs, tenderness, swelling, and edema around the joints, and the tips of the fingers may feel cold and cyanotic. The condition is triggered by diseases that are relatively common in the elderly, such as coronary insufficiency, angina pectoris, cerebrovascular disease, and calcific tendonitis. In mild cases, the condition tends to heal naturally, so rehabilitation, heat therapy, and steroid joint injections are performed.

肩の鈍痛のほか，運動が制限され，進行すると外旋運動が害される。痛みは頸部から腕にまで及び，手は赤くはれ，手指の伸展，屈曲が完全にはできなくなる。いわゆる交感性異栄養症の特徴である四肢末梢部の疼痛 (とうつう) ，灼熱感や，関節周辺部の圧痛，腫脹 (しゅちょう) や浮腫を伴い，手指末端は冷感，チアノーゼを呈することもある。冠不全，狭心症，脳血管障害，石灰化腱炎など比較的高齢者に多く見られる疾患が引き金となって起こる。症状の軽い例では自然治癒の傾向があるので，リハビリテーション，温熱療法，ステロイド剤関節注入などを行なう。
