Langerhans Islands - Langerhans Islands

They are also called pancreatic islets, or simply abbreviated as "islets of Langerhans." They are a group of endocrine cells scattered in island-like formations within the tissue of the pancreas. They were discovered by the 19th century German pathologist Langerhans, who named them islands (English) or Inseln (German). The diameter of the islets of Langerhans is approximately 50-200 micrometers, and their number in the entire pancreas ranges from approximately 200,000 to 2 million. Their total volume is said to account for 2% of the pancreas.

There are three types of cells in the islets of Langerhans: A cells (α (alpha) cells), B cells (β (beta) cells), and D cells (δ (delta) cells). A cells secrete a hormone called glucagon, which has the effect of increasing blood sugar. B cells are the most numerous, and secrete insulin. This hormone acts mainly on liver cells, muscle cells, and fat cells, making it easier to take in glucose, fatty acids, and amino acids from the blood, and is thought to be useful in the synthesis of glycogen, fat, and protein. When this hormone was first discovered, it was named after the fact that it is secreted from the islets (inula in Latin). Insulin works to lower blood sugar levels, and when blood sugar levels rise, insulin secretion is promoted. Therefore, diabetes occurs when secretion from B cells is impaired. Experimentally, diabetes can also be induced by selectively destroying B cells with alloxan or other agents.

D cells secrete a hormone called somatostatin, which inhibits the secretion of insulin and glucagon. Somatostatin-secreting cells are not only found in D cells, but also in the hypothalamus of the brain and the cells of the duodenal mucosa.

In recent years, immunohistochemical studies have revealed that cells called pp cells exist in the islets of Langerhans. pp cells produce a hormone called pancreatic polypeptide, and are thought to have various physiological functions, such as secreting gastric acid and pepsinogen, promoting bile secretion, and inhibiting intestinal peristalsis, but this is not yet known for certain.

膵島(すいとう)ともいい、単に「ラ島」とも略記する。膵臓の組織内に島状に散在する内分泌性細胞群で、19世紀のドイツの病理学者ランゲルハンスが発見し、島islands（英語）、Inseln（ドイツ語）と命名した。ランゲルハンス島の直径は約50～200マイクロメートルで、その数は膵臓全体で約20万個から200万個までとされている。また、その総体積は膵臓の2％を占めるといわれる。

　ランゲルハンス島の細胞群には3種類の細胞があり、A細胞（α(アルファ)細胞）、B細胞（β(ベータ)細胞）、D細胞（δ(デルタ)細胞）に分類される。A細胞は血糖を高める作用のあるグルカゴンというホルモンを分泌する。B細胞は細胞数がもっとも多く、この細胞からはインスリンが分泌される。このホルモンはおもに肝細胞、筋細胞、脂肪細胞に作用して血液中のグルコース、脂肪酸、アミノ酸を取り入れやすいようにし、グリコーゲン、脂肪およびタンパク質の合成に役だつと考えられる。インスリンの名は、このホルモンが最初に発見されたとき、「島（ラテン語ではinsula）から分泌される」ということから名づけられたものである。インスリンは血糖値を低める働きがあり、血糖値が高まるとインスリンの分泌が促進される。したがって、B細胞の分泌が障害されると糖尿病をおこす。実験的にも、B細胞をアロキサンなどで選択的に破壊すると糖尿病をおこすことができる。

　D細胞はインスリンやグルカゴンの分泌を抑制するソマトスタチンというホルモンを分泌する。ソマトスタチンを分泌する細胞はD細胞ばかりでなく、脳の視床下部や十二指腸粘膜の細胞にも存在する。

　なお近年、免疫組織学的に、pp細胞という細胞がランゲルハンス島に存在することがみいだされた。pp細胞は膵ポリペプチドというホルモンを産生し、胃酸とかペプシノゲンの分泌、胆汁分泌の促進、腸管の蠕動(ぜんどう)運動の抑制にかかわるなど、さまざまな生理作用をもつとされているが、まだ確実なことはわかっていない。
