Physiological lactation is only seen during lactation during the puerperal period, but non-physiological lactation occurs at other times. In many cases, elevated prolactin levels are observed in the blood. Causes include disorders of the diencephalon-pituitary system, pituitary tumors, ectopic prolactin-producing tumors, primary hypothyroidism, medications, trauma, and other chest diseases, but there are also cases where the cause is unclear, and these are called idiopathic galactorrhea. Prolactin (PRL), a hormone that stimulates milk secretion, is secreted from the anterior pituitary gland, but outside the puerperal period, it is secreted from the diencephalon (hypothalamus) and is suppressed by a prolactin secretion inhibitor that is transported to the anterior pituitary gland via the pituitary portal vein and acts there. 生理的な乳汁分泌は産褥(さんじよく)期の授乳中にのみみられるが,これ以外のときに非生理的乳汁分泌が起こる状態をいう。多くの場合に血中プロラクチン値の上昇が認められる。原因となる疾患には,間脳―脳下垂体系の障害,脳下垂体腫瘍,異所性プロラクチン産生腫瘍,原発性甲状腺機能低下症,薬剤,外傷その他胸部疾患などがあるが,とくに原因が明らかでないものもあり,これを特発性乳漏症という。乳汁分泌を促すホルモンであるプロラクチン(PRL)は,脳下垂体前葉から分泌されるが,産褥期以外は間脳(視床下部)から分泌され,脳下垂体門脈を経て脳下垂体前葉に運ばれて作用するプロラクチン分泌抑制因子によって抑制されている。
