In bones, bone cells, which are embedded in the matrix and control the metabolism of the matrix, receive nutrients from the abundant blood that circulates through the bones, but if blood circulation in the bone is widely interrupted for some reason, the bone cells die and bone metabolism stops. This condition is called avascular necrosis. When avascular necrosis occurs in the bone trabeculae that support the cartilage of the joint surface, the joint surface, which corresponds to the roof, is destroyed, just as the pillars of a house are damaged and the roof falls due to termite damage, causing the joint surface to become incompatible or deformed, resulting in the deterioration of joint function.

骨では，基質に埋まり基質の代謝をつかさどっている骨細胞は，骨を循環する豊富な血液から栄養を受けているが，骨の血行がなんらかの原因で広範に途絶すると，骨細胞は死滅し骨の代謝は停止する。この状態を虚血性骨壊死と呼ぶ。虚血性骨壊死が関節面の軟骨を支えている骨梁に発生すると，ちょうどシロアリに食害され家の柱が破損して屋根が落ちるように，屋根に相当する関節面が破壊され，関節面の不適合や変形を生じて関節機能の荒廃をきたす。
