Tacrolimus hydrate preparations may cause malignant tumors such as lymphoma, nephrotic syndrome, reversible posterior multifocal leukoencephalopathy (generalized convulsions, loss of consciousness, etc.), cerebrovascular disorder, diabetes mellitus, hyperglycemia, hemolytic uremic syndrome, thrombotic thrombocytopenic purpura, hemolytic anemia, ileus, mucocutaneous-ocular syndrome, dyspnea, progressive multifocal leukoencephalopathy, BK viral nephropathy, malignant tumors such as lymphoma, and pancreatitis.

Mycophenolate mofetil preparations may cause progressive multifocal leukoencephalopathy, BK virus nephropathy, pancytopenia, severe diarrhea, malignant lymphoma, acidosis, hypoxia, thrombosis, heart failure, arrhythmia, myocardial infarction, angina pectoris, pericardial effusion, pulmonary edema, convulsions, impaired consciousness, allergic reactions, and hearing loss.

Mizoribine preparations may cause bone marrow suppression, interstitial pneumonia, acute renal failure, Pancreatitis Water , hyperglycemia/diabetes, and skin disorders such as mucocutaneous ocular syndrome and toxic epidermal necrolysis may occur.

If any of these symptoms occur, discontinue use and notify your doctor immediately.

　アザチオプリン製剤は、ステロイド依存性のクローン病の緩解導入及び緩解維持、ステロイド依存性の潰瘍性大腸炎の緩解維持、ウェゲナー肉芽腫・結節性多発動脈炎・チャーグ・ストラウス症候群・大動脈炎症候群などの全身性血管炎、全身性エリテマトーデス、多発性筋炎、皮膚筋炎、強皮症、混合性結合組織病、難治性リウマチ疾患の治療にも用いられます。

　エベロリムス製剤は、心臓移植・腎移植における拒絶反応の抑制に用い、シクロスポリン製剤のネオラール及び副腎皮質ホルモン剤と併用します。

　また、シクロスポリン製剤は、全身型重症筋無力症（胸腺摘出後の治療において、ステロイドの使用で効果不十分、または副作用により使用困難な場合）、ベーチェット病、尋常性乾癬、再生不良性貧血、ネフローゼ症候群にも用いられます。

　タクロリムス水和物製剤は、全身型重症筋無力症、関節リウマチ（既存治療の効果が不十分な場合に限る）、難治性（ステロイド抵抗性、ステロイド依存性）の活動期潰瘍性大腸炎（中等症～重症に限る）、プログラフはループス腎炎、多発性筋炎・皮膚筋炎に合併する間質性肺炎の治療にも用います。

　グラセプター（0.5㎎、1㎎、5㎎）は、骨髄移植における拒絶反応及び移植片対宿主病の抑制にも用います。

　ミコフェノール酸モフェチル製剤は、腎移植後の難治性拒絶反応、ループス腎炎の治療にも用いられます。

　ミゾリビン製剤では、腎移植における拒否反応の抑制、ネフローゼ症候群、ループス腎炎、関節リウマチの治療に用いられます。

　血液中の白血球が減少し、感染に対する抵抗力が低下するので、いろいろな感染症にかかりやすくなります。医師から指示された検査は必ず受けてください。

①過敏症状（発疹、かゆみなど）、骨髄機能抑制（顆粒球減少、白血球減少など）、間質性肺炎、感染症、消化管潰瘍・出血・穿孔、肝機能障害・黄疸、腎機能障害などがおこることがあります。このような症状が現れたら、使用を中止して、ただちに医師に報告してください。

　アザチオプリン製剤では、リンパ腫などの悪性腫瘍及び重度の下痢、進行性多巣性白質脳症がおこることがあります。

　エベロリムス製剤では、移植腎血栓症、進行性多巣性白質脳症、BKウイルス腎症、血栓性微小血管障害、溶血性尿毒症症候群、血栓性血小板減少性紫斑病、悪性腫瘍、肺胞蛋白症、心嚢液貯留、高血糖・糖尿病の悪化、肺塞栓症、急性呼吸窮迫症候群などがおこることがあります。

　シクロスポリン製剤では、感染症、神経ベーチェット病症状（頭痛、発熱、意識障害、情動失禁など）、クリーゼ、進行性多巣性白質脳症（意識障害、認知障害、片麻痺へんまひ・四肢麻痺、言語障害など）、BKウイルス腎症、急性膵炎、血栓性微小血管障害、溶血性貧血、横紋筋融解症、悪性リンパ腫などがおこることがあります。

　タクロリムス水和物製剤では、リンパ腫などの悪性腫瘍、ネフローゼ症候群、可逆性後多巣性白質脳症（全身けいれん、意識障害など）、脳血管障害、糖尿病、高血糖、溶血性尿毒症症候群、血栓性血小板減少性紫斑病、溶血性貧血、イレウス、皮膚粘膜眼症候群、呼吸困難、進行性多巣性白質脳症、BKウイルス腎症、リンパ腫などの悪性腫瘍、膵炎などが現れることがあります。

　ミコフェノール酸モフェチル製剤では、進行性多巣性白質脳症、BKウイルス腎症、汎血球減少、重度の下痢、悪性リンパ腫、アシドーシス、低酸素症、血栓症、心不全、不整脈、心筋梗塞、狭心症、心膜液貯留、肺水腫、けいれん、意識障害、アレルギー反応、難聴などがおこることがあります。

　ミゾリビン製剤では、骨髄抑制、間質性肺炎、急性腎不全、膵炎すいえん、高血糖・糖尿病、皮膚粘膜眼症候群や中毒性表皮壊死症などの皮膚障害がおこることがあります。

　このような症状がおこったら、使用を中止して、ただちに医師に報告してください。

②食欲不振、吐き気、出血、下痢、味覚異常、言語障害、視覚障害などがおこることがあります。

①錠剤とカプセル剤があり、薬により、初めは1日体重1㎏当たり2～3㎎（成人で1日2～3錠）を服用し、経過をみながら減量していくなど、こまかな調整が必要です。1日の使用回数と使用時間・1回の使用量については、医師の指示をきちんと守ってください。

　エベロリムス製剤は、1日2回の服用で、食事の影響を受けるため、食後または空腹時のいずれかの一定の条件下で使用し、定期的に検査を受けてください。

②あらかじめ問診の際に、持病・アレルギーなどの体質・現在使用中の薬の有無を医師に報告するとともに、使用前に薬の効果と副作用について医師・薬剤師からよく説明を聞き、注意事項をきちんと守ってください。

　とくに、白血球減少症や重い感染症にかかっている人は病気がより悪化します。また、腎・肝障害や出血しやすい病気をもつ人、高齢者などは、使用量を減らすなどの、医師による対策が必要になります。

　以上のような条件をもつ人は、あらかじめ医師に報告してください。

③妊婦、妊娠している可能性がある人は、あらかじめ医師に報告してください。薬によっては使用できないことがあります。

④これらの薬の使用中は禁酒を守ってください。

⑤これらの薬を使用中に、ほかの薬を使う必要が生じたときは、必ず医師に相談してください。使用中は生ワクチンを利用した予防接種は受けられません。

　アザチオプリン製剤では、フェブキソスタット製剤を使用中の人には使用できません。

　シクロスポリン製剤では、タクロリムス（外用剤は除く）、ロスバスタチン、ボセンタン、アリスキレン、アスナプレビル、バニプレビル、ピタバスタチン製剤を使用中の人、コルヒチンを使用中で肝・腎障害のある人には使えません。また、授乳中の人も使用できません。

　タクロリムス水和物製剤では、シクロスポリン、ボセンタン水和物製剤、スピロノラクトン製剤やトリアムテレン製剤などのカリウム保持性利尿剤を使用中の人は使えません。腎障害や感染症のある人は、医師に相談してから用いてください。

　また、エベロリムス製剤やシクロスポリン製剤、タクロリムス水和物製剤を服用する際には、グレープフルーツの摂取を避けてください。
