One of the peptic ulcers in which the digestive action of gastric juice is an important factor. It occurs when the balance between the attacking factor of gastric juice secretion and the defensive factor of mucosal resistance is disrupted. For example, when the attacking factor becomes stronger due to mechanical stimuli such as alcohol, coffee, and spices, or stress, and the defensive factor becomes weaker due to illness, blood circulation disorders, stress, etc., the balance is disrupted. Furthermore, in recent years, Helicobacter pylori has been found in patients with gastric and duodenal ulcers, and this bacterium is also considered to be an important factor. It mostly occurs on the lesser curvature side. It is more common in men over 40 years old (2 to 3 times more common than in women). It is accompanied by weight loss, constipation, fatigue, and anemia, and pain is felt from the epigastrium to the left subcostal arch 20 to 30 minutes after eating. It is confirmed by occult blood reaction in stool, X-ray examination, gastroscopy, etc. Although it does not progress to gastric cancer, differential diagnosis is necessary because the site of onset and symptoms are similar. In the past, the treatment was based on medical therapy, especially dietary therapy, but surgery was performed in cases of severe bleeding, perforation, pyloric stenosis, suspected cancer, and cancerous ulcers. In addition, patients were now given a high-calorie diet earlier than before, which shortened the healing period.

胃液の消化作用が重要な因子となる，消化性潰瘍の一つ。胃液分泌の攻撃因子と粘膜の抵抗力の防御因子のバランスがくずれることによって生じる。たとえば，アルコール，コーヒー，香辛料などの機械的な刺激や，ストレスなどで攻撃因子が強くなり，病気，血行障害，ストレスなどで防御因子が弱くなると，バランスはくずれる。さらに近年，胃・十二指腸潰瘍患者にピロリ菌（ヘリコバクター・ピロリ）が認められることから，この菌も重要な因子と目されている。ほとんど小彎(わん)側に発生する。40歳以上の男性に多い（女性の２〜３倍）。体重減少，便秘，疲労感，貧血を伴い食後20〜30分にみぞおちから左肋骨弓下に疼痛(とうつう)を認める。便の潜血反応，Ｘ線検査，胃カメラ等によって確診される。胃癌に移行することはないが，発生部位ならびに症状が近似するため，鑑別が必要である。内科的療法とくに食事療法を主とするが，大出血，穿(せん)孔，幽門狭窄(きょうさく)，癌化の疑いある場合および癌性潰瘍に対しては手術を行う。なお食事は以前よりも早期に高カロリー食をとらせるようになり，治癒期間も短縮された。
