Anticoagulant - anticoagulant

Drugs that prevent blood clotting are also called anticoagulants or blood coagulation inhibitors. They are used to prevent diseases in which blood clotting is abnormally increased, such as pulmonary embolism and venous thrombosis, and are also used during hemodialysis, blood transfusions, and blood tests. Blood clotting occurs through the following mechanism: thromboplastin in the blood is activated by calcium ions, turning prothrombin into thrombin, which then acts on plasma fibrinogen to turn it into fibrin. Fibrin then entangles with other substances to form blood clots. Therefore, the mechanisms of anticoagulants are as follows:

(1) Substances that bind with and remove calcium ions: sodium citrate and ethylenediaminetetraacetic acid (EDTA).

(2) Substances that inhibit the production of prothrombin: warfarin potassium and dicumarol of the warfarin series, and phenylindazine of the indazine series.

(3) Agents that inhibit the action of thrombin: heparin sodium, heparin calcium, dextran sulfate preparations, and heparinoids.

(4) Fibrin dissolving agents: Enzyme preparations such as streptokinase and streptodornase.

Sodium citrate and heparin are used to prevent blood clotting during transfusions, and in blood products such as stored blood. Heparin is also used to prevent blood clotting during surgery and hemodialysis, as well as for thrombosis and embolism. Warfarin is taken orally to prevent and treat thrombosis and embolism. Dextran sulfate, as a sodium salt, is injected to treat dyslipidemia and enhance the effects of anticancer drugs. Heparinoids include Hirudoid ointment, which promotes peripheral vascular circulation, and is used to treat and prevent thrombophlebitis and keloids.

血液の凝固を阻止する薬剤で、抗凝固薬あるいは血液凝固阻止剤ともいう。肺塞栓(そくせん)や静脈血栓などの血液の凝固性が異常に亢進(こうしん)している疾患を防止したり、血液透析や輸血、血液検査時に用いられる。血液凝固は次のような機序（メカニズム）による。すなわち、血液中のトロンボプラスチンがカルシウムイオンによって活性化され、プロトロンビンからトロンビンとなり、このトロンビンが血漿(けっしょう)フィブリノゲンに作用してフィブリンとする。フィブリンは絡み合って血栓をつくる。したがって、抗凝血薬の機序としては次のようなものがある。

(1)カルシウムイオンと結合してこれを除去するもの　クエン酸ナトリウム、エチレンジアミン四酢酸（EDTA）。

(2)プロトロンビンの生成を阻害するもの　ワルファリン系のワルファリンカリウムやジクマロール、インダジオン系のフェニルインダジオン。

(3)トロンビンの作用を抑制するもの　ヘパリンナトリウム、ヘパリンカルシウム、デキストラン硫酸製剤、ヘパリノイド。

(4)フィブリンを溶解するもの　ストレプトキナーゼ、ストレプトドルナーゼなどの酵素製剤。

　クエン酸ナトリウムやヘパリンは輸血時の血液凝固阻止、または保存血など血液製剤に用いられる。ヘパリンはそのほか血栓・塞栓症に、また手術時や血液透析時の血液凝固阻止を目的として使用される。ワルファリンは血栓・塞栓症の予防と治療に内服で使用される。デキストラン硫酸はナトリウム塩として脂質異常症、制がん剤作用増強などに注射で用いられる。ヘパリノイドには末梢(まっしょう)血管循環促進作用を有するヒルドイド軟膏(なんこう)があり、血栓性静脈炎やケロイドの治療と予防などに用いられる。
