Antipyretic - Antipyretic

Also written as antipyretics. A drug that lowers body temperature to normal when it becomes abnormally high, commonly known as a fever reducer. It acts on the temperature control center in the central nervous system, especially the hypothalamus, to lower body temperature, and many of them have a mild analgesic effect. It is used as a symptomatic treatment for feverish diseases and is often used to treat colds, as well as for headaches, toothaches, neuralgia, and rheumatism, taking advantage of its analgesic and anti-inflammatory effects. There are salicylic acid, aniline, pyrazolone, and others. Cinchona bark is the world's oldest used antipyretic, but its active ingredient, quinine, is no longer used for that purpose at all today.

The most well-known salicylic acid is aspirin, with salicylamide and ethenzamide often used in over-the-counter cold medicines. Sodium salicylate, choline salicylate, salsalate, and aluminum aspirin are aspirin derivatives that cause less stomach damage. Acetaminophen and phenacetin are well-known aniline drugs, but they can cause kidney and liver damage. Flufenamic acid, mefenamic acid, and diclofenac sodium are also aniline drugs that have strong anti-inflammatory effects and are used as an analgesic, antipyretic, and anti-inflammatory agent to treat rheumatoid arthritis, frozen shoulder, and other conditions. Pyrazolone drugs include antipyrine, aminopyrine, sulpyrine, and isopropylantipyrine. When taken orally, aminopyrine becomes a nitroso compound in the intestinal tract, which is carcinogenic, and is no longer used much except as a suppository. Sulpyrine is water-soluble and can be used for intramuscular injections, but it is tissue damaging and has become a problem as one of the causes of quadriceps shortening. Isopropylantipyrine is used to relieve headaches, migraines, and neuralgia, and phenylbutazone and oxyphenbutazone, like flufenamic acid and mefenamic acid, are also called nonsteroidal anti-inflammatory drugs and are used to treat various inflammatory diseases such as rheumatoid arthritis. Other phenylacetic acid derivatives such as ibufenac, ibuprofen, and indomethacin are also frequently used as antipyretics, analgesics, and anti-inflammatory drugs.

下熱薬とも記す。体温が異常に上がったとき、これを下降させ平熱にする薬剤で、俗に熱さましともいう。中枢神経系とくに視床下部にある体温中枢に作用して体温を降下させるもので、多くは軽度の鎮痛作用を有している。熱性疾患の対症療法として用いられ感冒の治療に繁用されるほか、その鎮痛作用や消炎作用を利用して頭痛、歯痛、神経痛、リウマチにも常用される。サリチル酸系、アニリン系、ピラゾロン系その他がある。世界でもっとも古くから使用された解熱薬はキナ皮であるが、その有効成分であるキニーネは現在その目的ではまったく使用されていない。

　サリチル酸系でもっとも有名なのはアスピリンで、そのほかサリチルアミド、エテンザミドは一般用薬としての感冒薬の組成としてよく使用されている。サリチル酸ナトリウム、サリチル酸コリン、サルサラート、アスピリンアルミニウムはアスピリンの胃障害を少なくしたものである。アニリン系ではアセトアミノフェンとフェナセチンが有名であるが、腎(じん)障害や肝障害をおこすことがある。またアニリン系のフルフェナム酸、メフェナム酸、ジクロフェナックナトリウムは抗炎症作用が強く、鎮痛、解熱、消炎剤として関節リウマチ、四十肩、五十肩などの治療に用いられている。ピラゾロン系ではアンチピリン、アミノピリン、スルピリン、イソプロピルアンチピリンがあり、アミノピリンは内服により腸管でニトロソ化合物となり、これが発癌(はつがん)性を有することから坐薬(ざやく)のほかはほとんど使用されなくなった。スルピリンは水溶性で筋肉注射用にも使用されるが、組織障害性を有し大腿(だいたい)四頭筋短縮症の原因の一つとして問題となった。イソプロピルアンチピリンは頭痛、片頭痛、神経痛の鎮痛の目的で配合され、またフェニルブタゾン、オキシフェンブタゾンはフルフェナム酸、メフェナム酸などと同じく非ステロイド系消炎剤ともいわれ、関節リウマチなど各種炎症性疾患の治療に用いられている。そのほかフェニル酢酸系のイブフェナック、イブプロフェンやインドメタシンも解熱・鎮痛・消炎剤として繁用される。
