As its name suggests, this snail has a left-handed shell (illustration). It is a large snail with a shell 3cm high and 4.5cm wide. It coils about five and a half times, and the surface of the shell of those that live on plains is yellow with black bands around it, and the navel hole on the underside is also black. However, those that live in mountainous areas are black-brown with wide bands above and below the black bands around it. This type is called the brown left-handed snail E. q . montium , and the small individuals that live on Hekura Island are called the Hekura snail E. q . heguraensis .

名のように殻が左巻きのオナジマイマイ科の巻貝（イラスト）。殻は高さ3cm，幅4.5cmの大型のカタツムリ。約5回半巻き，殻の表面は平地にすむのは黄色でまわりに黒い帯があり，また下側のへそ穴のところも黒い。しかし，山地にすむのは黒褐色でまわりの黒い色帯の上下にも幅の広い色帯ができる。この型をチャイロヒダリマキマイマイE.q.montium，舳倉（へくら）島にすむ小さい個体はヘクラマイマイE.q.heguraensisという。
