A nobleman from the end of the Heian period to the beginning of the Kamakura period. He was the eldest son of Fujiwara no Tsunemune, the Minister of the Left. As the eldest son of a ministerial family, he rose steadily through the ranks, becoming the Minister of the Right in November 1198, and six months later, the Minister of the Right. In 1203, despite having a wife with whom he had spent 30 years, he married Kyonii, who had recently lost her husband. Kyonii was a close aide to the retired Emperor Gotoba, and was a central figure in the Imperial Court at the time, wielding power as she pleased. Through this marriage, Yorizane was able to make his daughter Reishi (Inmeimon'in) the Empress of Emperor Tsuchimikado, and also gained the position of guardian of the retired Emperor Gotoba. Important political decisions in the Imperial Court were made by the retired Emperor, Yorizane, and his wife, and it is said that he was a kind of regent as the political adviser to the retired Emperor.

平安末・鎌倉前期の公卿。太政大臣従一位。左大臣藤原経宗の長男。大臣の家の嫡子として順調に昇進を遂げ,建久9(1198)年11月右大臣となり,半年後,太政大臣となった。建仁3(1203)年,30年を共に過ごした妻がいるにもかかわらず,夫を亡くしたばかりの卿二位と結婚した。卿二位は後鳥羽上皇の側近の女房で,当時の朝廷の中心的存在であり,ほしいままの権勢を振るっていた。この結婚によって,頼実は娘麗子(陰明門院)を土御門天皇の皇后にすることができ,また後鳥羽上皇の後見という立場を得ることができた。朝廷の重要な政治決定は上皇と頼実夫妻の3人でなされており,上皇の政治の顧問として関白のような存在であったという。
