Intercostal neuralgia

This disease is characterized by sudden onset of pain that radiates forward from the vertebrae along the ribs, and the pain subsides relatively quickly but recurs. The intercostal nerves control the movement of the intercostal muscles and the sensation of the skin in the anterior and lateral chest. They run along the ribs, and the fifth to ninth intercostal nerves are easily affected. There are cases of primary intercostal neuralgia where the cause cannot be found, but there are many cases of secondary intercostal neuralgia caused by various diseases such as shingles, rib and spinal diseases, spinal tumors, aortic aneurysms, pleurisy, and heart diseases. In secondary intercostal neuralgia, the pain increases with posture, coughing, deep breathing, and other chest movements, and treatment of the underlying disease is necessary, and painkillers and vitamins are administered symptomatically. In the case of primary intercostal neuralgia, intercostal nerve blocks may be performed using local anesthetics such as lidocaine or alcohol. By the way, intercostal nerve blocks are not only used to treat intercostal neuralgia, but are also sometimes used as auxiliary anesthesia for upper abdominal surgery and anesthesia for chest wall surgery.

急激におこり、椎骨(ついこつ)より肋骨に沿って前方に放散する痛みを特徴とする病気で、痛みは比較的短時間で治まるが反復する。肋間神経は肋間筋の運動および前胸部や側胸部の皮膚の知覚を支配している神経で、肋骨に沿って走行しており、第5～第9肋間神経が侵されやすい。原因がみいだせない原発性肋間神経痛もあるが、帯状疱疹(ほうしん)、肋骨・脊椎(せきつい)疾患、脊髄腫瘍(しゅよう)、大動脈瘤(りゅう)、胸膜炎、心臓疾患など種々の病気が原因となっておこる続発性肋間神経痛が多い。続発性肋間神経痛では体位や咳(せき)・深呼吸など胸郭運動によって疼痛(とうつう)が増強し、原因疾患の治療が必要で、対症的には鎮痛剤やビタミン剤などが投与される。原発性肋間神経痛の場合には、リドカインなど局所麻酔剤あるいはアルコールによる肋間神経ブロックが行われることもある。ちなみに、肋間神経ブロックは肋間神経痛の治療ばかりでなく、ときに上腹部手術の補助麻酔や胸壁手術の麻酔に用いられることもある。
