Also called endemic osteoarthritis. A type of endemic disease common in eastern Siberia and northeastern China. Suspected cases have been reported in Yamaguchi, Tokyo, and Chiba in Japan, but there is no conclusive evidence. The disease was named after Soviet physicians N. Kashin and E. Beck. It is common among the poor and elderly. It begins insidiously in childhood and progresses gradually, affecting mainly the small joints of the fingers and toes, and gradually affecting the large joints. In many cases, joints become swollen and deformed, and bow legs, knock-knees, incomplete dislocations, and spontaneous fractures are also seen. Pain is present and worsens in spring and autumn. Causes include excessive intake of iron, vitamin A deficiency, and heavy metal poisoning, but there is also a theory that it is due to toxic substances produced by the decomposition of plants being contained in drinking water, and the true cause is unknown. 地方病性変形性骨関節炎ともいう。東部シベリア,中国東北地方に多い地方病の一種。日本でも山口,東京,千葉で疑わしい症例が出ているが確証はない。ソ連の医師 N.カシンと E.ベックの名を取ってこの病名がつけられた。貧困者,高齢者に多い。小児期に潜行的に起り,徐々に進行し,主として手足の指の小関節が侵され,次第に大関節にも及ぶ。多くは関節の腫脹や変形が生じ,O脚,X脚,不完全脱臼,自然骨折もみられる。疼痛があり,春秋に悪化する。原因は鉄分の過剰摂取,ビタミンAの欠乏や重金属の中毒などがあげられているが,植物の分解で生じる有毒物が飲料水に含まれるためという説もあり,真の原因はわかっていない。
