There are subjective and objective abnormalities, but in the narrow sense it refers to objective abnormalities, where abnormal sensations occur due to a disorder in the sensory conduction pathway from the peripheral organs to the cerebral sensory areas. For example, normal touch but impaired temperature and pain sensations are seen in spinal cord diseases and leprosy. Conversely, touch sensation is impaired while temperature and pain sensations are preserved, as seen in peripheral nerve trauma and neuritis. Partial loss or dullness of pressure sensation is seen in various diseases of the spinal cord and cerebral cortex, cerebral palsy, etc. Force sensation disorders are also seen in diseases of the cerebellum.

自覚異常と他覚異常があるが，狭義には他覚異常をさし，末梢器官から大脳知覚領に至る知覚伝導路の障害により異常感覚が起こること。たとえば，触覚が正常で温度覚と痛覚が障害されるのは脊髄疾患，ハンセン病などにみられる。これとは逆に触覚が障害され，温度覚と痛覚が保たれるのは，末梢神経の外傷や神経炎にみられる。圧覚の部分的消失・鈍麻は脊髄，脳皮質等の各種疾患，脳性小児麻痺などによる。また力覚障害は小脳の疾患にみられる。
