Joint fusion surgery - kansetsu koteijutsu

This is a surgery to fix a joint in a good position so that it cannot move, thereby making it functionally advantageous. For example, in cases where the upper limbs cannot be raised at all due to deltoid muscle paralysis caused by spinal polio, shoulder joint fixation surgery can be performed to enable the upper limbs to be raised, or in cases where the patient is unable to walk due to pain caused by hip joint disease, the hip joint can be fixed in a good position to relieve pain, increase support, and enable walking.

The surgical method involves removing the articular cartilage and performing a bone graft to obtain bony ankylosis in a good position. After surgery, the patient is immobilized in a plaster cast and wears a brace until the bone heals completely.

関節を動かないように良肢位に固定し、それによって機能的に有利にしようとする手術をいう。たとえば、脊髄(せきずい)性小児麻痺(まひ)（ポリオ）の三角筋麻痺で上肢がまったくあがらないような場合に、肩関節固定術によって上肢をあげられるようにするとか、股(こ)関節の疾患で疼痛(とうつう)のため歩行できないような場合に、股関節を良肢位に固定して疼痛を除き、支持性を増して歩行を可能にするなどである。

　手術方法は、関節軟骨を切除し、骨移植を行って良肢位で骨性強直を得させる。手術後、骨が完全に癒合するまではギプス固定、装具の装用などを行う。
