Alternans - kogomyaku

Japanese: 交互脈 - こうごみゃく

This refers to the phenomenon in which the arterial pulsation becomes stronger and weaker with each beat, or the wave height increases and decreases with each beat on an electrocardiogram. There are mechanical alternans and electrical alternans, and both can be seen at the same time. Mechanical alternans is the phenomenon in which the arterial pulsation becomes stronger and weaker with each beat. It occurs when fluid accumulates in the pericardium, causing the heart to float and perform pendulum motion, or when the cardiac contractility is weakened, causing the cardiac output to increase and decrease with each beat. It is one of the symptoms of severe cardiac dysfunction.

動脈の拍動が一拍ごとに強弱すること，あるいは心電図上，一拍ごとに波高が高低する現象をいう。機械的交互脈と電気的交互脈があり，両者が同時に認められることもある。機械的交互脈とは，動脈の拍動が一拍ごとに強くなったり弱くなったりする現象で，心囊に液体が貯留して心臓が浮遊状態となって振子運動をする場合や心収縮力の弱った場合に心拍出量が一拍ごとに増減するために起こる。心機能が著しく障害された症状の一つである。
