Health insurance run by health insurance associations. Employers establish associations either alone or jointly, and their employees become insured persons. Whereas the insurance premium rate is a fixed rate under the Japan Health Insurance Association (formerly government-managed health insurance), this rate is calculated based on the standard monthly salary and standard bonus amount, and can be set independently (generally low) according to the rules of each association. Also, while insurance premiums are split between the employer and the insured under the Japan Health Insurance Association, this allows the employer to pay a higher share of the premium.
